**NPAAMB Indigenous Youth Employment & Training is committed to providing urban Indigenous youth, who reside within its service district areas covering parts of Southern Ontario, with employment and training services.**

**Purpose**:Working as part of a team in a fast paced environment, the primary function of this position is to provide wrap around case management services to urban Indigenous youth by developing a plan of care that is community oriented, family centered, strengths based, and reflects the cultural values and practices of each youth. The project outcome is that “youth know about and easily navigate resources in their communities”.

**RELATIONSHIPS**

**Internal**
- Reports to and communicates with the Manager of Direct Services for direction and approvals to ensure project is meeting desired outcomes and time lines.
- Works with all staff, particularly the Youth Service Officers and Youth Success Mentors to carry out the project goals.
- Actively participates in NPAAMB program and staff meetings.

**External**
- Builds and maintains positive partnerships and effective working relationships with project partners.
- Works with other potential partners, as required to ensure project objectives are met.
- Promotes and raises awareness of the project throughout the urban serviced communities by NPAAMB.

**GENERAL JOB REQUIREMENTS**
- Prepare and oversee facilitation of a plan of care that addresses the social issues impacting urban Indigenous youth
- Ensure the wraparound process is completed with high conformity to the model and track progress toward goals
- Coordinate and facilitate client wrap around meetings
- Encourage and help urban Indigenous youth, who have difficulty navigating community programs and services
- Develop and maintain client files inclusive of all required documents and maintain accurate case notes
- Research community resources and make referrals to these service organizations
- Ensures that all required confidentiality requirements are met
- Participation in mandatory wrap around training
- Arrange payments for child care, transport
