**About us**:
St. Mary’s Home (SMH) is a not-for-profit charitable organization providing specialized residential and community support services to marginalized young pregnant women (ages 12-25), young parents and their children. The approach is holistic, grounded in a strong commitment to strengthen and build upon the personal resilience of each individual.

**Position Summary**:
The Bilingual Case Manager (CM) provides support to the specialized shelter program and reports to the Direct Service Manager. The CM role is to implement the Mission Statement of SMH by providing Support and Case Management to at-risk pregnant / parenting youth and their infants who are living in our specialized shelter program. Within a strengths-based philosophical framework, the CM promotes positive dynamics, defuses negative dynamics and is a positive role model.

The CM oversees the stay of residents on their respective case load, the transition to outreach after residential service support advocates for residents and liaises with our Young Parent Outreach Centre and multiple service partners. CM ensures resident files meet annual Licensing and accreditation standards and ensures continued excellence in meeting or exceeding clinical and operational standards.

With a parent/infant attachment focus, this role provides goal-specific services to a caseload of SMH participants by employing participant-centric, strengths-based and trauma-informed practices. Additionally, this role involves the implementation of the SafeCare outreach program at SMH. SafeCare is an evidence-based program for parents and their children under the age of 5. It focuses on reducing risk factors for neglect, abuse, and unintentional injury through three modules Health, Safety, and Parent-Child Interaction. The program is an outreach-based program, delivered in participant’s homes.

**Responsibilities**:

- Maintain comprehensive, accurate, and up-to-date documentation, including record keeping, client files, reports and service coordination and case closure documentation;
- Engages the participants one-on-one in an effective and positive manner and within a Collaborative Problem-solving framework;
- Oversees dynamics within a Crisis Prevention Intervention (CPI) framework;
- Facilitate emergency intakes and admissions and oversees waitlist for residential service;
- Implement assessments using approved evidence-based tool;
- Complete appropriate internal and external referrals based of participant identified needs in their plan of care;
- Oversee participants interface with other Community Services, Public Health and Child Welfare;
- Respond to participant mental health and addiction recovery, family violence and trauma-related challenges;
- Provide support to participants with Conflict Resolution, Crisis Support and Residential Guideline Compliance;
- Oversee Bedroom Standards, assisting participants with hands-on support to meet requirements for fire, health, safety and licensing;
- Assists participants in meeting their attendance requirements for appointments and programs;
- Oversee discharge planning and post-discharge referrals and linkages;
- Participate in ongoing professional development as required by SMH;
- Participate actively in staff meetings:

- Provide coaching with Primary Care Staff on Collaborative Problem Solving and Conflict Resolution skills building
- Supports an integrative approach within the agency between the Residence and the Young Parent Outreach Centre
- Engage in regular supervision and support
- Cover when fellow CM is not available
- Promote positive morale among team members and agency well-being'
- Other duties as required.

**Qualifications**:

- Bachelor’s degree in Social Work or related field
- 4 years supportive counseling / social work experience directly related to substance use, mental health, family violence and/or trauma issues and infant mental health.

**OR**
- Diploma as an CYW, SSE or related equivalent
- 2 years of progressive experience working directly related with at risk youth, mental health, family violence, trauma and infant mental health
- Experience in a Residential Setting is an asset
- Experience working with pregnant and parenting youth is an asset

**Required Knowledge and Skills**:

- Excellent communication and interpersonal skills
- Ability to work independently and also as a member of a team
- Capacity to provide service in both official languages is required
- Experience with Motivational interviewing
- Certificate and applied knowledge of Crisis Prevention Intervention is an asset
- Knowledge of adolescent pregnancy and parenting issues
- Knowledge of Infant Mental Health and Attachment is a strong asset
