Rehabilitation Assistant, Forensic Mental Health

**Job Summary**:
In accordance with the Mission, Vision and Values, and strategic directions of Provincial Health Services Authority patient safety is a priority and a responsibility shared by everyone at PHSA. As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Under the general supervision of the assigned leader, the Rehabilitation Worker, Forensic Mental Health, implements established programs, develops and evaluates new comprehensive individual and group rehabilitative programs within clinical streams to meet the specific needs of patients using psychosocial principles and practices that facilitate the patient in a successful reintegration into the community. Operates the Centre’s passenger transportation vehicle to drive residents to designated locations.
**Duties/Accountabilities**:
2. Works collaboratively with the interprofessional team to develop/implement individualized rehabilitative treatment plans for patients by identifying patient strengths, limitations and functional level for program involvement. Works with team members in the regular evaluation of assigned programs with regard to patients’ assessed needs and progress, and modifies or changes programs as indicated, e.g., recommending suitability for programs. Utilizes skills and knowledge to assess a patients condition and make in the moment adjustments to interventions in response to the patients current health status.
3. Completes rehabilitation assignments within designated scope of practice and documents patient progress. Reports observations of patients’ behavior and progress to therapist and Treatment Team. Communicates concerns and/or changes to patients’ condition. Prepares individual discharge reports to meet department/organization documentation standards at the conclusion of each closed program by summarizing information focusing on patients’ life skills and independence levels and recommending appropriate community supports where available. Maintains outcome and workload measurement statistics.
4. Works collaboratively with other Rehabilitation colleague, therapists and Treatment Team members to analyze the needs of target patient populations as a basis for new program development, standards and evaluation strategies by reviewing relevant literature and suggesting interventions that will support the achievement of desired treatment outcomes. Participates in the development of specific program curricula to address the functional level of the target population including researching, obtaining, and creating appropriate educational, testing, and other program materials to meet established program goals and objectives and evaluation processes under the guidance and supervision of the designated therapist
5. Maintains work area and equipment by ensuring equipment and supplies, such as tools, materials and chemicals used in the program are stored appropriately and accounted for, arranging repairs as required, and assuming the responsibility for the purchase of supplies to support programs as required. Cleans, disinfects and maintains equipment including mobility self-care aids and adaptive devices, and reports malfunctioning equipment to appropriate staff. Fabricate/repair adaptive devices such as splint straps under the direction of the designated therapist.
6. Performs regular security functions to maintain the safety and security of patients, staff, families and the general public at all times and takes immediate action to mitigate risks, as well as reporting all real and potential risk issues. Ensures safety and security of the staff, patients and general public during all planned activities such as Staff Supported Community Outings.
7. Drives residents to designated locations as scheduled, by operating the Centre’s passenger van/bus with a seating capacity of 11 to 25 passengers, including the driver. Assists clients with embarking and disembarking the vehicle. Conducts inspections of vehicle by checking oil, gas and fluid levels, tire condition and ensuring that lights and signals are functioning; cleans interior and exterior of vehicle after use; and maintains records of distances traveled, gas expenditures and inspections. Recommends vehicle and equipment repairs as required.
8. Provides support to volunteers/students during activity programs and demonstrates related techniques as required. Participates in orientation of new client/residents, staff and volunteers as directed by the designated therapist
9. Participates in team meetings, continuing education, conferences, and/or corporate initiatives by methods such as providing input, presenting relevant material, representing Rehabilitation Services on organizational committees and working groups, acting as an advocate for the goals and objectives of the program, and promoting optimal support for clients and the team.
10. As per established procedure, oversees the collection and receipt of cash for items
