**RAI Coordinator**
**Hours of work** : 30 hours per week
**Responsibilities**:
- Promote the emotional and physical well-being of residents by optimizing the Home’s funding through the coordination of Resident Assessment Instruments (RAI) and Minimum Data Set (MDS)
- Ensure the information derived from the assessments are used in the formulation of care plans tailored to satisfy the requirements of individual residents
- Generate and review RAI-MDS Output Reports with the leadership team and front line staff and facilitating their use in clinical and operational decision-making.
- Monitor and provide on-site support to all RAI-MDS users at Markhaven.
- Ensure that all registered staff involved in MDS process complete the AIS examination annually and keep records to that effect
- Partner with the Director of Care to provide the home with the services provided by the Ontario Telemedicine Network.
- Effectively communicate with staff at all levels, residents, families, other departments, and external stakeholders
**Skills and Experience**:
- Current RN or RPN certificate of competency from the College of Nurses of Ontario
- Previous RAI-MDS training, certification and annual recertification in keeping with CIHI standard and annual AIS certification of competency with a pass mark of 85 or higher
- Work experience as RAI Coordinator in a long term care environment
- Knowledge of CIHI’s processes
- Completion of supplemental training for RAI Coordinators as offered through the Ministry of Health and CIHI.
- Experience auditing resident MDS assessments
- A minimum of three years clinical, project leadership or management experience
- Experience with Ontario Telemedicine Network an asset
