Huntington Society of Canada (HSC) is a leading Canadian charity dedicated to a world free from Huntington disease (HD). Our passionate and collaborative Family Services team is in search of a Child and Youth Care Practitioner, who can design and launch a comprehensive Child, Youth, and Young Adult program within HSC, tailored to the needs of young people affected by Huntington Disease. This position reports to the National Director of Family Services. This is a hybrid, full-time contract position (with the possibility of extension), reporting at least weekly to head office, located in Waterloo. The Child and Youth Care Practitioner will be working with our Family Services Team to support children, youth, and young adults throughout our national program.

**Key Accountabilities**:
The overall goal of this role is to create a dynamic, responsive, and supportive environment for young people to grow, connect, and receive the necessary resources for their well-being as they navigate their journeys through Huntington Disease.

This role focuses on developing and implementing the Child, Youth, and Young Adult Program; a structured program to support children, youth, and young adults (ages 10-35) within the organization. The position requires strategic planning, program design, curriculum development, and engagement strategies to ensure effective support and positive outcomes for participants.

**Key Responsibilities include**:

- **Program Development**:Designing and launching a comprehensive Child, Youth, and Young Adult Program at HSC, tailored to the needs of young people, inclusive of evaluation processes, advocacy, promotion, and data analysis.
- **Position Scope**:Focus on children, youth, and young adults, within the context of families, the community and their life space. Practice is grounded in trauma-informed, relational, anti-oppressive, participatory, and a strengths-based approach.
- **Survey Design & Data Gathering**:Creating surveys to collect valuable insights that will shape curriculum and engagement strategies.
- **Facilitation**:Lead and facilitate at least two support groups, one social support group and one psychoeducational workshop annually to provide emotional and educational assistance. Produce/edit one Fact sheet annually.
- **Community Collaboration**:Working with both internal and external partners to strengthen program impact.
- **Case Planning & Coordination**:Contributing to cross-disciplinary teamwork to enhance service delivery.
- **Family System Navigation**:Assisting families in accessing relevant support resources.
- **Data Management**:Documenting interactions with participants in the organization’s database to track engagement and outcomes. Maintain detailed records of client interactions.
- **Youth Mentorship Program**: Leadership, oversight, delivery and growth of the pre-existing Youth Mentorship Program within HSC. Recruit, train, and support mentors/mentees. Deliver ongoing training to participants.

**Travel Responsibilities**:

- This position requires at least weekly travel to the HSC national office, located in Waterloo, Ontario.
- Local travel to meet with clients, families, and to in-person groups and events.
- Remote work is anticipated.
- Travel to, and participation in, national team meetings (twice per year) and the biennial HSC national conference (held at various sites across Canada), as scheduled and required.

**Requirements**:

- Diploma in Child and Youth Care or acceptable equivalent required
- Post secondary academic programs in Child and Youth care an asset
- Eligible for certified membership in the Ontario Association of Child and Youth Counsellors (OACYC) required
- 5 + years’ experience working with children, youth, young adults, and families required
- 3 + years’ experience training and supporting volunteers or staff required
- Experience with Program Development required
- Demonstrated excellent teamwork and interpersonal skills
- Knowledge of Huntington disease (HD)
- Demonstrated professionalism in dealing with confidential information and situations
- Experience working with client databases/documentation databases
- Working knowledge of Microsoft Office 365 and products
- Vulnerable Sector Check conducted within the last 6 months required
