**Position Profile**:
The nCourage, Client Navigator is responsible for acting as the point of first contact for potential clients and community service providers. The Client Navigator conducts triage, intake, screening and assessments; provides intervention, crisis support and advocacy to individuals, and makes appropriate referrals to community services for individuals and their families. The incumbent also establishes and maintains a relationship with service partners and referral sources. The Client Navigator is responsible for conducting virtual and in-person outreach activities.

The Client Navigator is responsible for acting as the point of first contact for potential clients and community service providers. The Client Navigator conducts triage, intake, screening and assessments; provides intervention, crisis support and advocacy to individuals, and makes appropriate referrals to community services for individuals and their families. The incumbent also establishes and maintains a relationship with service partners and referral sources. The Client Navigator is responsible for conducting in-person services, virtual as required, as well as outreach activities.

**Role Overview**:
**Responsibilities**:

- Complete intake and/or triage, and assessments to determine client needs, interests.
- Support the implementation of next steps for care.
- Keep careful and accurate records and maintain client case file of all relevant information pertaining to assigned caseload, via the designated database.
- Conduct therapeutic, trauma-informed risk assessment and safety planning.
- Support identifying the client’s needs, and desired referrals to on-site and off-site partners for service, in coordination with the case managers.
- Coordinate with case managers to support continuum of care and access to services.
- Short-Term and/or supportive case management when required
- Coordinate and develop the onsite calendar of service partners for clients to access.
- Develop and coordinate onsite appointments at the hub.
- Address all inquiries regarding services and support.
- Track and complete monthly data and statistical reports based on funding requirements.
- Participate in care conferencing and consultations with nCourage team, and/or service providers.
- Consult and report any risk concerns to appropriate agencies.
- Provide case management as assigned.
- Provide support with intake process to service providers in the community, and promote awareness and understanding of the program and services offered at nCourage.
- Engage in collaborative partnerships with PHTSPC agencies and other partners as identified to support seamless service.
- Coordinate nCourage in person services onsite, ensuring appointments are confirmed and coordinated daily as part of the continuum of care for victims and survivors.
- Participate in and facilitate committee meetings, as assigned.
- Advocate for clients and report barriers and gaps as identified by clients.
- Other duties as assigned.

**Community Involvement**:

- Positively and professionally represent the nCourage Collaborative in the community by participating and collaborating with community partners on various projects and initiatives as requested.
- Maintain current and up-to-date knowledge of community resources.
- Present/Group Facilitation of programming for community partners, or those at risk as needed/assigned.

**Administration**:

- Compile statistical information and prepare reports as required for program evaluation, and updates.
- Support client data collection by regularly auditing and reviewing the data prior to reporting due dates.
- Maintain detailed and appropriate documentation, record keeping, progress notes, and care path plans, within assigned databases.
