**Job Description**:
As a
**Registered Dietitian in** the Renal Program you will:

- Assess nutritional status and needs of medically complex renal patients in all renal outpatient programs including our satellite units, and subsequently implement the appropriate nutritional care plan.
- Monitor the effectiveness of the nutrition care plan and provide education and discharge planning for patients, as indicated
- Participate as an integral member of the interdisciplinary health care team in the provision of exceptional patient care
- Act as a resource for nursing, allied health and physicians and participate in relevant hospital policy/procedure and program development
- A Registered Dietitian with a Bachelor’s degree with a Dietitians of Canada (DC) approved internship
- Several years of clinical experience preferred
- Demonstrated interest in furthering education in renal
- Professional Affiliation: Registered with the College of Dietitians of Ontario. Eligible for membership in the Dietitians of Canada and appropriate DC networks.
