**Position Description**:

Reporting to the Manager, Patient and Family Collaborative Support Services, the Indigenous Peer Support role provides formalized peer support in services of the MHAP for individuals who identify as Indigenous living with mental illness and/or addiction through one-to-one work, peer driven groups, advocacy and role-modeling. To advocate with and represent the voice of individuals with the lived experience of mental illness and/or addiction both internal to and external to SJHH.

**QUALIFICATIONS**
- Individuals with experience working within the Indigenous communities are encouraged to apply
- Completion of Grade 12 high school diploma
- Certificate in Peer Support Training from Recognized Training Provider and or relevant work experience with demonstrated ability and understanding of core essentials of peer support provision preferred
- Minimum 1-year of related experience (volunteer or employed) providing peer support in a mental health and/or addiction setting
- Experience and knowledge of trauma-informed care considered an asset
- Personal lived experience of mental illness and/or addiction
- Lived experience with Indigenous worldviews, cultures, and values
- Demonstrated ability to role-model strong self-care and self-management strategies in recognition of the need for health, personal-growth, and resiliency in peer support work
- Knowledge and understanding of the Urban Indigenous Community as well as Six Nations of the Grand River Territory, and Mississaugas of the Credit Communities
- Familiarity and understanding of serious mental illness/addiction and mental health systems preferred
- ASIST or similar Suicide Intervention Training preferred
- Experience/training in group facilitation preferred
- Demonstrated knowledge and practiced skills in providing culturally safe and responsive mental health services to clients of diverse ethno-racial, gender, and cultural backgrounds is preferred

**RESPONSIBILITIES**
- Work one-to-one with individuals to support them in the development and achievement of self-determination, personal empowerment, and their individual recovery goals
- Support and work in collaboration with the individual and their clinical team members upon request, in the creation of recovery goals and or treatment plan
- As required, facilitate and coordinate peer driven/recovery-oriented groups utilizing the values and principles of a peer support/person-centred philosophy
- Travel to meet individuals in the community and utilize public transportation when travelling with individuals
- Maintain ethical and professional conduct in accordance with Psychosocial Rehabilitation Canada and Peer Support Accreditation Certification Canada in keeping with their respective codes of ethics and practice guidelines
- Participate in ongoing professional development and continuing education including, but not limited to, attending workshops and conferences, specific professional training and certification events (e.g. WRAP, ASIST) to assist in peer support work
- Collaborate with community partners, service providers and other stakeholders to help facilitate connections and referrals to other resources whenever appropriate
- Organize/coordinate community outings for the purposes of researching culturally-relevant community-based resources such as educational, counseling, financial support, social agencies etc., in order to support an individual in their own goals
- Complete appropriate recordkeeping/documentation for health record, and statistical data entry
- In partnership with other Program staff, co-facilitate educational and awareness presentations regarding mental health and recovery-oriented issues, peer support and departmental services (e.g., family support group, facilitated education for learners, orientation, etc.)
- Act as a staff/departmental representative on internal and external committees as required/requested by Manager
