Transitional Aged Youth Worker

**What you will do in the role**:
- Working with youth from ages 12-23 that are in that are on various types of agreements, supporting youth in their successful transition out of the child welfare system into adulthood.
- Develop the Pathway Plan’s in collaboration with the Children Services Department in order to set/ meet goals that will assist them in their transition to adulthood.
- Monitor the youth’s progress in meeting the goals that were set in the youth pathway plan.
- Collaborating with Children’s Services Workers in supporting youth that are in care and residing in room and board arrangements and independent living arrangements.
- Commitment to providing services in ways that respect First Nations cultural and spiritual practices.
- Knowledge of case management systems, reporting procedures and responsibilities and the ability to work within those systems.
- Effective written and oral communication skills, problem solving, advocacy and assessment skills.
- Ability to manage crisis and adverse situations with staff, youth, and families.
- Ability to work within a multi-disciplinary team, as well as manage and maintain public inquiries.
- Knowledge and understanding of health and safety practices related to outreach work and Universal Care precautions.
- Knowledge and understanding of the Child, Youth and Family Services Act, and other relevant legislation.
- Ability to work within the policies, standards, procedures and the vision, mission, and core values of Kunuwanimano Child and Family Services.

**What you bring to the table**:
- Diploma in Child and Youth Worker or Bachelor of Social Work with previous experience within First Nations communities.
- Experience in working with culturally appropriate service models for First Nations people.
- Demonstrate excellent verbal and written communication skills to effectively consult with clients and a variety of internal and external professionals, the community, agencies, and organizations.
- Able to provide on-call services when required.
- Must be willing to work flexible hours.
- Must be willing to travel and cover the entire District and have access to a reliable vehicle with a $2 million rider liability.
- Must produce clear Criminal Record Check with Vulnerable Sector Screening.
- Must possess a valid “G” Ontario driver’s licence.
