Ontario Health atHome is seeking an **Clinical Lead, South West Regional Wound Care Program**

Opportunity Summary:
The South West Regional Wound Care Program (SWRWCP) advocates for the integrated seamless, timely and equitable delivery of high quality, safe, efficient, and cost effective, person-centered, evidence-informed skin and wound care to the people of Ontario Health atHome, regardless of the setting. The SWRWCP is hosted by Ontario Health atHome South West region, but supports all hospitals, long term care (LTC) homes, contracted community nursing service provider agencies and Primary Care Providers. The SWRWCP advocates for Ontario Health atHome health system partners to become leaders in integrated skin and wound care management. As a member of the Quality team, the Clinical Lead for the SWRWCP ensures integrated delivery of wound care that spans the continuum of care within Ontario Health atHome South West. In collaboration with health system partners, the Clinical Lead formulates and executes a coordinated strategy for the delivery of wound care based on leading practice. **This is a temporary full time, two (2) month opportunity open to any of our South West office locations (London, St Thomas, Stratford, Woodstock or Owen Sound).**

While some elements of the role are currently being performed remotely, the successful incumbent will be required to regularly report to their assigned office location.

What will you do?
- Oversee and promotes the ongoing development and maintenance of a sustainable knowledge translation framework to support all care providers across all health care settings.
- Promotes integration, coordination and service innovation in developing strategic and change management plans, and implementation work plans for program staff.
- Ensures the development and implementation of cross-organizational wound care education and resources including evidence informed guidance and tools, expert resources and education events, expert wound care specialists and Wound Care Champions to enhance health service provider quality and skill improvement capacity to advance skin and wound care management and improve clinical outcomes. Identifies initiative-specific educational needs of partners and staff, and provides learning opportunities to promote knowledge to action.
- Meets with senior leaders to establish program objectives and ensures projects meet objectives, are timely and cost efficient, patient focused and align with the vision of the SWRWCP and Ontario Health atHome Integrated Health Service Plan (IHSP)
- Promotes the ongoing alignment of wound product selection through Ontario Health atHome
- Supports the development and delivery of annual work plans based on priorities set by Ontario Health atHome focused on, but not limited to, effective prevention and management of wound care across the system of care.
- Participates on the Ontario Health atHome multidisciplinary planning tables and committees, representing SWRWCP as it relates to the IHSP and system improvements. Works collaboratively with the Ontario Health atHome to address program needs and achieve targets
- Maintains a cross sector partnership governance structure (SWRWCP Strategic Steering Committee) and reporting processes to support the SWRWCP and promote collaboration and investment in improving and integrating wound care.
- Establishes and maintains strong relationships and partnerships with South West health service providers across four health care sectors and 100+ organizations (primary, acute, long term care and community) as well as other regional and provincial and national key stakeholders with respect to program deliverables, planning and opportunities for collaboration. Establishes and identifies capacity and program plans in conjunction with system leaders.
