Vitality Assessments Group is a community-based rehabilitation practice that provides qualified and experienced therapists to serve in their homes, schools and other community settings. We focus on assessments and treatment of individuals and families whose lives have been affected by trauma and various injuries. We serve pediatric, adult and older adult populations.

**We are looking for an experienced Social Worker to join our team of skilled rehabilitation professionals in the Ottawa area. The Social Worker will work as a part of an interdisciplinary team to facilitate the client's recovery and long-term goals.**

**Duties and Responsibilities**:

- Emotional Support - facilitate the client’s resolution of challenges in their lives through improved coping and skill development.
- Service Linkage - facilitate the connection of a client with another agency in order to strengthen the network of support.
- Service Coordination - ensure an integrated and collaborative approach from all participating agencies to maximize our client's recovery.
- Service Negotiation - negotiating the removal of barriers, limiting a client's rehabilitative efforts, in order to gain access to a service or improve the services that may be provided by an agency
- Resource Mobilization - supporting the client in obtaining the resources necessary for daily living.
- Client Advocacy - providing support in order to make sure the client’s best interest is considered. This involves advocating on our client's behalf to ensure they have access to resources, services and benefits. Furthermore, it involves advocating for the Social Work profession and educating individuals on the benefits of Social Work as a part of rehabilitation and treatments.
- Education and Training - providing opportunities and information for clients to gain new skills, or improve existing ones. and to empower our clients.

**Qualifications and Skills**:

- A Bachelor's or Master's of Social Work
- Motivated self-starter with superior technical, verbal and written communication skills
- Understanding of the code of ethics, OCSWSSW guides/regulations, and appropriate documentation skills.
- Great interpersonal skills and a team player
- Current registration with the OCSWSSW with liability insurance.
- Driver’s license with access to a vehicle
- Additional languages are considered an asset.

**Job Types**: Full-time, Part-time, Permanent
Part-time hours: 20-35 per week

**Salary**: $65,000.00-$90,000.00 per year
