**The Opportunity**
We are looking for a **Peer Navigator** to join our **Connect 2 Care (C2C) Team** at CUPS.
The C2C Peer Navigator works with the Graduation and Health Navigators to assist C2C clients who are ready to transition from intensive team supports to lighter touch case management supports or independence in the community. As a person with lived and living experience, you will support clients by providing emotional and social support through dialogue and shared experience while working to build trust and rapport with clients.
You will provide safe, quality patient-centered support while reflecting the shared mission, vision, and values of CUPS. The goal of this role is to support integrated care within CUPS and build the capacity of CUPS clients to achieve independence in the community.

- **Reports to**: Team Lead, C2C & CAMPP
- **Contract type**:Permanent
- **Schedule**: Part-time, Mon to Fri, **32**hours per week
- **Starting salary**:$55,906 - $58,673
**Key Responsibilities**
- Establish trusting and open relationships with clients.
- Support integrated care within CUPS as well as warm transfers and transitions to community based social and medical supports that align with clients’ goals.
- Support clients to connect with and strengthen natural supports in the community (friends, family, neighbors, faith groups).
- Build client’s capacity to navigate and access appropriate community-based resources that meet their needs.
- Provide peer coaching through modelling healthy habits and partnering in clients’ recovery goals.
- Advocate for client needs and guide multi-disciplinary care teams on effective and respectful approaches to working with clients living with the vulnerability of poverty or homelessness.
- Perform duties both alone and with colleagues in a variety of settings including in private homes, shelter, on the street, in camps, in office.
- Ensure all required data is entered accurately into organizational databases, document all appointment notes and referrals provided.
- Provide culturally aware support to CUPS clients, celebrating the diversity of clients and being aware of power dynamics of privilege and oppression that may be present.

**Employment Requirements**:
- Life experience in addiction/mental health/homelessness/poverty
- Class 5 Driver’s License with drivers abstract and personal vehicle available to use for work purposes & participant transport
- Basic First Aid and CPR Certification
- Police/RCMP Background and Vulnerable Sector Check

**Work Conditions**
- Exposure to trauma and emotionally complex cases
- May be exposed to infectious waste, diseases, conditions, etc., including exposure infectious diseases.
- Manual dexterity required to use desktop computer and peripherals.
- Intermittent physical activity including walking, standing, sitting, lifting and supporting clients.
