**Position Overview**:
Reporting to the Manager, Workplace Health and Safety (WHS), the Disability Management Associate's role is to support and facilitate the return to work of ill or injured employees to their highest level of function through collaboration with the Workplace Health & Safety team, managers, union and external healthcare providers in compliance with legislation, hospital policy and cost avoidance (i.e. grievances, arbitrations and hearings). The incumbent will employ the principles of disability management to actively manage the cases of all disabled employees to ensure the employees are provided with appropriate case management.

**Responsibilities**:

- Short Term Disability (STD) Case management activities, conduct case assessments, contacting the employee to explain the disability management process and gather information for assessment purposes.
- Develop and document an understanding of the situation and the factors supporting as well as inhibiting a successful return to work (RTW) for the employee.
- Communicate adjudication decision to all stakeholders and maintain regular communication with all stakeholders regarding status of STD claim pertaining to an absence or an accommodation.
- As appropriate, corresponding or meeting with management, employees and unions if applicable to support case management activities.
- Determine an appropriate RTW goal with the employee and the employer, provide active support for the return-to work planning. Facilitate the Gradual Return to Work (GRTW) planning process assisting with documentation (pertaining to restrictions) and communication to all parties as required
- Communicate proactively with the employer’s Human Resources (HR) representative regarding any case management issues that may influence the workplace and RTW planning.
- Coordination of and participation in case conferences with the Occupational Health Physician and other members of the Workplace Health and Safety team.
- Maintain appropriate correspondence related to case management decisions/activities in Parklane reporting system.
- Accurate and timely computer data entry related to disability management and accommodation to ensure the file is up to date at all times.
- Preparation of employee files and case summaries for grievance, arbitration and/or legal files
- Other projects and tasks as assigned.

**Requirements**:

- University/College Degree or Diploma in a Health Sciences or Business Field preferred
- Completion of disability focused program or previous experience with disability management as asset
- Minimum 3 years working experience, preferably in a hospital setting
- Demonstrated proficiency with Microsoft Outlook, Word, Excel, Power Point, and Parklane
- Demonstrated strong planning, organizational, and coordination skills
- An ability to manage multiple priorities with mínimal direction
- Excellent interpersonal skills, ability to work with staff and clients in a collaborative team environment
- Demonstrated satisfactory work performance and attendance history

**Asset**:

- Completion of a medical terminology program
- Previous experience with disability management
- Hospital experience
