**Job Description - Admissions & Case Coordinator - Full-Time Contract (18 Months)**

**Location**: Inpatient Unit (227 Victoria Street, Toronto, Ontario)
**Status**: Full-Time Contract (18 Months)
**Hours**: 37.5 hours per week
**Union/Non**: This is a unionized position
**Rate of pay**: $33.20 per hour

**Position Summary**

The Admissions and Case Coordinator position will ensure that Youthdale values are reflected in the ongoing triage of services to identified need, transition, and continued recovery of the children, youth, and young adults. This role will assist the Youthdale in intake, intake data collection and input, admissions, case management, and admission requirements for all Youthdale programs.

The Admissions and Case Coordinator will support a workplace that embraces equity, diversity, inclusion and belonging, encourages teamwork and complies with all applicable regulatory and legislative requirements.

**Accountability**

The position reports directly to: Interim Director, Clinical Programs.

**Corporate Responsibilities**
- Fosters positive and constructive interpersonal relationships with the staff, volunteers, and residents, visitors, outside agencies, government officials and related professional associations.
- Maintains and adheres to all relevant legislation.
- Maintains and adheres to organizational policies and procedures.
- Works in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contravention’s of the Act, in a timely manner.
- Development, implementation, monitoring and evaluation of training and development programs aligned with the organization’s strategic priorities and guiding principles.
- Demonstrates management practices, which are consistent with the guiding principles and philosophy of Youthdale Treatment Centres.

**Responsibilities**:

- Complete all intake functions as it relates to Court-Ordered and Emergency admissions to the ASU, as well as admissions into the Live in Treatment programs (including Step-up/Step-down), TAU, and Day Treatment programs
- Gather collateral and referral information to present at prioritization and bed flow meetings, and facilitate a discussion to ensure acceptance decisions are made for clients who meet the program criteria
- Utilizes standardized tools to aid with admission/decline decision making such as the CALOCUS and INTER-RAI
- Collaborate with client and caregiver to ensure their voice is incorporated in the process
- Assist with determining the type of admission to the ASU: Court Order or Emergency admissions
- For Court Ordered admissions, assist families with the paper work required to obtain a court order, and assist with securing funding if needed
- For clients who are declined - ensure they are triaged to appropriate services
- Ensure documentation standards are met and data collection as per policy guidelines
- Coordinate and support appropriate services for clients and their families while at Youthdale
- Verbal communication and writing documentation that addresses client’s care and treatment. Coordinating meetings and appointments as mandates and/or needed
- Consult with inter-professional teams to determine client needs
- Facilitates clients' utilization of appropriate community services by performing duties such as researching available services, liaising with these agencies, providing related advice and consultation to clients and providing referrals to appropriate community treatment alternatives and services. Utilizes available community resources to develop supportive and rehabilitative programs for adults and their families.
- Represent Youthdale in a professional manner during external community meetings
- Ensure standards of professional practice with respective regulatory bodies (Ontario College of Social Workers and Social Service Workers, MCCSS, MOH, Auditor, Accreditation, etc.) are met
- Maintain and adhere to all relevant legislation, organizational and occupational health and safety policies and procedures, and the mission, vision, and philosophy of the agency.
- Understanding the CYFSA and ministry licensing requirements as it relates to Child and Youth Services
- Maintain clients records and prepare reports

**Qualifications**
- College diploma in related field
- Regulated Health Care Professional and registration with the OCSWSSW, or related professional association preferred
- At least two years of experience such as intake/triage/case management
- Knowledge of community resources within the GTA and step down interventions
- Knowledge of children and youth mental health and the social determinants of health.
- Expert knowledge of system navigation and referral pathways to community mental health, addiction support, health, social agency resources, etc.
- Strong interpersonal and communication skills.
- Ability to communicate verbally and write clear, concise reports and documentation.
- Clinical experience is required to support decision-maki
