- Coordinate and manage individualized care plans for clients.
- Communicate with clients, families, and caregivers to ensure consistent, high-quality care.
- Schedule caregivers and maintain accurate staffing records.
- Monitor client satisfaction and address any concerns promptly.
- Develop and implement local marketing initiatives to attract new clients.
- Build and maintain relationships with referral sources, community organizations, and healthcare professionals.
- Ensure compliance with company policies and industry regulations.

**Qualifications**:

- Health care professional (RN/RPN/LPN/Social Worker/Occupational Therapist/Physiotherapist).
- Previous experience in home care coordination and case management of the elderly.
- Strong understanding of senior care needs and caregiver management.
- Knowledge of marketing and community outreach strategies.
