We currently have an opportunity for a temporary full time **Project Lead, ALC & Patient Flow **. This is a temporary position and anticipated to extend to October 31, 2025.

**How Project Lead, ALC & Patient Flow our teams at Grand River Hospital**:

- Conducts best practice reviews and site visits to establish the applicable patient flow improvements and support program evaluation and benchmarking
- Provides expertise in ALC strategies and provides education related to ALC to various service providers
- Identifies the project structure and key resources to participate in the patient flow project
- Establishes and disseminates the ALC escalation policy and supports escalation processes
- Leads the team in the execution of the policies, processes and practices for patient flow
- Engages and supports planning with the Ontario Health at Home and other key external stakeholders to establish alternative models of care for patients
- Considered the expert in the organization on ALC and patient flow
- Influences areas without out direct accountability to ensure flow of patients occurs
- Educates the organization on ALC and the designation, discontinuation and appropriate use ALC
- Works with the physicians and care team to assist with the patient and family meetings regarding ALC and discharge

**At Grand River Hospital, the Project Lead, ALC & Patient Flow brings the following**:

- An undergraduate degree/diploma in a related health discipline, graduate degree in related discipline preferred
- Regulated health professional (e.g. RN, RPN, OT, PT, etc.)
- A minimum of 5 years in a leadership position the healthcare environment, preferably in a hospital setting
- Familiarity with the various legislation impacting the discharge planning function and alternative levels of care (ALC)
- Understanding of the complex discharge planning function and roles and responsibilities across the care team and various stakeholders.
- Experience leading professional staff, including coaching, fostering consensus decision-making, and creating collaborative, productive team-based work environment for change initiatives to achieve quality patient care
- Demonstrated knowledge of performance measurement, quality improvement and risk management; ability to identify risks, intervene effectively and escalate appropriately
- Excellent consultation, facilitation, negotiation, problem solving, consensus building/team building and collaboration skills with an exhibited ability to gain timely consensus and alignment of multiple impacted and interested parties
- Demonstrated ability to take complex ideas and translate them for teams to drive action, accountability, and consensus
