**Overview**

**Title**: Manager - London Middlesex CSS Central Intake

**Status**: Permanent Full-Time

**Hours of Work**: Monday to Friday; 37.5 hours per week

**Starting Rate of Pay**: Starting pay rate 32.69/hr.

Reporting to the Director of Community Support this position manages the day to day operations of our London Middlesex Lead Agency work. This demanding role provides leadership and direction to a team of Central Intake Coordinators, Assessors and Community Services Planners while building strong relationships with the community partners and other health care organizations.

**Role Requirements**
- Possess a post-secondary school diploma/degree and a minimum three years’ experience with an in-home/community care organization or social/health service program
- Knowledge of the London Middlesex Community Support Services providers is an asset
- Ability to provide high-quality customer service in a fast-paced environment with multiple priorities and competing deadlines
- Demonstrated problem solving, assessing situations, setting priorities and critical thinking skills
- Demonstrated ability managing and developing teams
- Demonstrated ability in multi-sectoral collaboration and community development
- Excellent presentation and public speaking skills
- Excellent interpersonal and communication skills; both written and oral
- Demonstrated proficiency using the Microsoft Office suite of programs, including Word, Excel, and PowerPoint
- Valid Driver’s License, insurance and use of a vehicle
- Must possess (or able to acquire) a current Police Vulnerable Sector Check
- Understanding of InterRAI preliminary Screener and InterRAI CHA Assessment is an asset

**Role Responsibilities**
- Working as the first point of contact, the CSS Central Intake Manager will work closely with the Central Intake Team and system partners to streamline process, improve effectiveness and facilitate strong relationships within the community
- Communicates with funders, clients and care teams and coordinates with program staff to process referrals, initiate services, implement service delivery changes, and address/ respond to phone inquiries from community, customers, clients, doctors, service providers and other agencies
- Strengthens teams by providing individualized coaching and performance management
- Receive and Coordinate all standardized documentation, data collection, triaging of calls.
- Prepares and sends out client service agreements, consents and program information.
- Works collaboratively and maintains positive relationships with partner agencies
- Accountable for being knowledgeable of all policies and procedures related to their employment and functions.
- Supports the leadership team by providing insight and clarity on program needs
- Maintains statistical reporting of service as required by the funder
- Performs other tasks as assigned

- A valid Canadian Driver’s License and have regular access to a reliable vehicle
- Valid automobile insurance with liability coverage
- Ability to work Monday to Friday 8:30am till 4:30pm
