The Ontario government is building a connected health care system centered around patients, families and caregivers. These changes will strengthen local services, making it easier for patients to navigate the system and transition between providers.

Ontario Health Teams are a new way of organizing and delivering care that is more connected to patients in their local communities. There are 58 Ontario Health Teams across the province providing full provincial coverage and ensuring that every person in Ontario can have the support of an Ontario Health Team. The goal is to ensure that everyone in Ontario can benefit from better coordinated, more integrated care.

Children’s Centre Thunder Bay is the administrative support partner to the Noojmawing Sookatagaing Ontario Health Team.

The OHT is currently seeking to fill the following position:
Clinical Lead - OHT

Part Time Contract Position - approximately 7 hours per week

Annual Salary $60,000

15% in lieu of benefits, statutory holiday & vacation pay

**Responsibilities**:
As the Clinical Lead for the OHT, you will play a vital role in advancing an integrated care system. In this role, you will collaborate with the OHT operations team and the Co-Chairs of the Primary Care Network providing clinical leadership, guidance, and promotion in the design, development, and implementation of an integrated care system in Noojmawing Sookatagaing OHT.

This role requires a seasoned clinical professional with strong leadership, communication, and organizational skills to ensure the successful execution of initiatives that improve healthcare delivery and patient outcomes in the District and City of Thunder Bay. A commitment to health equity and an understanding of the multiple facets that encompass integrated care is essential.

**Qualifications**:
The position of Clinical Lead requires:

- Primary Care Provider: Licensed Physician or Nurse Practitioner
- Valid Driver's license and ability to travel within the City, District and region.
- Ability to work flexibly based on project needs.
