**Support Group Facilitator**

**Job Summary**:
Under the direction of the Manager of Education and Community Programs, the Support Group Facilitator will be responsible for planning, leading and evaluating support group sessions for Persons Living with Dementia and their Care Partners. The Support Group Facilitator will create a compassionate, respectful, and structured setting where participants can share experiences, receive emotional support, and education within an inclusive environment. The role of the Support Group Facilitator will also fulfill all expectations outlined in agency Policies and Procedures.

**Essential Duties and Responsibilities**
- Facilitate Dementia-focused support groups in-person, and virtual, on a regularly scheduled basis within chapter district
- Provide emotional support, guidance, and education for Care Partners and Persons Living with Dementia
- Guide discussion, ensuring equal opportunity for participation while creating a safe and inclusive space
- Monitor group dynamics and address conflicts or emotional situations with empathy and professionalism, seeking supervision when necessary
- Maintain confidentiality and uphold ethical standards in all interactions
- Maintain accurate documentation and statistical data in accordance to organizational policies
- Collaborate with team members to ensure groups align with organizational goals, and client needs
- Participate in education sessions when available to maintain professional development
- Ability to recognize when a client requires additional support and offer appropriate resources
- Assist with outreach, recruitment, and promotion of support groups
- Actively participate in organizational meetings as required

**Other Duties**
- Other duties as required, consistent with the job classification

**Education**:

- Post-secondary education in Social Work, Psychology, Nursing or related field with a comprehensive knowledge and understanding of Alzheimer Disease and related dementias

**Experience**:

- Demonstrated experience facilitating support groups, workshops or peer-support programs
- Comprehensive knowledge of Alzheimer’s disease and related dementias and their impact on the person, care partner, family, and care providers
- Experience with older adults, Dementia care, or caregiver support
- Strong interpersonal and communication skills with the ability to engage diverse participants
- Demonstrated ability to work independently and collaboratively with team members and community partners
- Knowledge of trauma-informed, anti-oppressive, and culturally sensitive practices
- Strong understanding of community resources and referral pathways
- Bilingual communication skills (English/French) encompassing both verbal and written proficiency

**Travel Requirements**
- Valid Ontario Driver’s License, good driving record, and current auto insurance
- Travel across Chapter District as required.
- No special physical demands are required beyond the performance of general office duties.
- Sensory attention is required for most of the workday (looking at a computer screen, reading documents etc)
