Social Worker, Long Term Care

Reporting to the Director of Care, the Social Worker provides psychosocial assessment, counselling/psychotherapy, case

management, resident/family education, supportive counselling, service coordination of admissions and consultation

according to organizational and best practice standards in order to assist residents to achieve optimal quality of life. The

Social Worker provides leadership in high risk social situations, program development, advocacy, and developing and

maintaining strong community linkages. The Social Worker aligns his/her work with the operational priorities related to

long-term care legislation and regulations and is accountable for maintaining the Code of Ethics and Standards of Practice of

the Ontario College of Social Workers and Social Service Workers.***

**Role Responsibilities**
- Prioritizes referrals, assessments, admissions and interventions based on identified domains and urgency
- Performs resident assessment utilizing population-specific and evidence-based assessment strategies
- Advocates for, and arranges family/community support team conferences as necessary, in consultation with the
- resident/POAPC and Care Team
- Evaluates outcomes of Social Work treatment plan on an ongoing basis and modifies as appropriate, completing follow
- up as needed and reflecting activities in resident health record and RAI-MDS
- Provides psycho-education and support around the resident illness/functional outcomes and the family’s role in the
- therapeutic process
- Facilitates community referrals in collaboration with the Care Team (this includes but is not limited to community
- agencies such as Home and Community, SMH, BSTU, Alzheimer Society etc.)
- Provides leadership in care navigation when supporting review and completion of Admission Agreement with
- resident/POAPC, while articulating additional and preferred care and services
- Takes a primary role in understanding and conveying resident/family/provider expectations about long-term care as well
- as what to expect in relation to care and services in relation to legislation and regulations
- Facilitates access to care and transitions by actively supporting predictive discharge planning, where applicable
- resources/services
- Builds and maintains linkages with appropriate community agencies to facilitate effective collaboration related to access

to care and transitions

**Required Qualifications and Experience**:

- Bachelor of Social Work (MSW) from a recognized educational institution
- Current unrestricted registration with the Ontario College of Social Workers and Social Services Workers
- Two years of experience in providing Social Work service in an institutional setting with the relevant population,
- preferred
- Experience with electronic patient record systems, preferred
