**Position Overview**

The Complex Case Worker ensures that newcomer clients of all backgrounds are equipped with the necessary information and supports to engage with the community systems in New Brunswick including but not limited to health care, government programs and supports. This position also ensures clients with low English skills access specialized healthcare beyond the scope of what is effectively provided by Tele Care or other government services. This position also ensures casual/contracted interpreters and volunteers are scheduled to support clients at external appointments. This position works collaboratively with the Settlement Counsellor Team Lead, casual interpreters and volunteers to meet client needs. This position is also required to make follow up phone calls to clients and translate short written documents from time to time. Evening and weekend work may be required on occasion.

**Core Responsibilities**
- Conduct holistic assessments of client strengths and needs, developing and implementing appropriate case plans for high need/complex case Government Assisted Refugees o ensure clients are referred to specialists immediately post arrival.
- Provide supportive counselling, referrals and information to empower clients to solve and cope with challenges arising from refugee resettlement in Canada
- Arranges access for clients and families to all relevant health care appointments, including organization of interpreter services and transportation arrangements both within and outside NB
- Maintain client referrals to the Refugee Clinic for initial health care check-ups
- Collaborate with Health Working Groups to identify and address gaps in service to all RAP clients in Community Health, Public Health and Mental Health;
- Orient clients and Health care providers to IFH documents, as required;
- Connect all our RAP clients to PAHA- Post Arrival Health Assessment.
- Work with Health Liaison from the Settlement team to identify, assist and follow up with clients in need of Heatlh in all community contexts including but not limited to healthcare, government programming, community supports, etc. with the goal of empowering clients as much as possible;
- Coordinate health appointments with RAP clients;
- Coordinate and connect RAP clients with Canadian red cross and others health care providers as required when disability equipment and mobility issues.
- Follow client families with high health or complex needs
- Deliver frequent information and orientation sessions regarding local hospitals, health clinics and other access points to care;
- Orient clients to prescriptions, as required;
- Orient clients to emergency contacts, as required;
- Foster strong relationships with healthcare providers, pharmacies, dentists and opticians in the region;
- Provide meaningful resources to clients regarding health and well-being, including mental health;
- Assist Newcomer Connections staff with other duties during peak service delivery times;
- Provide insight about the Newcomer community’s needs and challenges with respect to healthcare;
- Help maintain current client records in CRM, iCare and other relevant databases;
- Complete monthly expenses;
- Attend staff meetings;
- Any other duties as assigned by RAP Supervisor and/or Manager, Newcomer Support Services and/or GM of Newcomer Connections.

**Skills and Qualifications**
- A Bachelor’s Degree, Community College diploma or similar from an accredited institution;
- Must be fluent in a second language, with preference for Arabic & Somali.
- Heightened sensitivity to the needs of our immigrant community;
- A minimum of 2 years work ex
