Nanaimo Brain Injury Society Client Support Worker

This post is for the purpose of covering a maternity leave up to 12 months. Part-time (28hrs/week) $27.57/hour + benefits (after 3 month probationary period)
**RESPONSIBILITIES**
**Job Purpose**
The Client Support Worker will provide intake, person-centred support, timely information and guidance to assist individuals recovering from acquired brain injury. They will work closely with health professionals, social services, voluntary and community sector organizations to ensure a continuity of care and seamless service delivery. The ability to facilitate positive working relationships with communities, local voluntary and sector partners is essential, as is the capacity for teamwork.

**Client Support**
- Respond to referrals and community calls, and conduct intake interviews as required.
- Ensure all necessary forms and documentation relating to intake, including consent is complete and all necessary follow up actions are taken.
- Schedule and carry out client follow-up within 2-3 business days.
- Assist the Navigator Housing Support Worker with client case needs/supports.
- Refer clients and community to appropriate community support services, both within NBIS and outside of the organization.
- Understand the resources and services available on Vancouver Island specific to the brain injury population, and help clients connect to appropriate supports as needed.
- Provide transitional supports during period of exceptional client need.
- Offer one-to-one supports that can assist people in their emotional, social and practical adaptation to life following acquired brain injury.
- Develop and maintain positive working relationships with other key organizations and institutions that support persons with brain injury.
- Ensure information on the Navigator Program is communicated accurately to clients and community partners.
- Work collaboratively with all NBIS program coordinators to ensure outcomes are achieved, and provideconsultation and support to co-workers as needed.
- Assist with education workshops and presentations for clients, caregivers and allied service professionals as requested by management.
- Attend all meetings, conferences and training programs as requested and agreed by NBIS.
- Represent the agency in a professional manner.
**Facilitation**
- Support virtual peer weekly drop-in as needed, track participants and provide reports.
- Develop, deliver and evaluate weekly social program, schedule presenters, create program materials, register and communicate with participants.
- Coordinate, register and track attendance for all activities related to caregivers.
**General/ Administrative**
- Practice appropriate record keeping duties using NBIS shared drives and Case Management Database.
- Assist the Navigator Housing Support Worker with mentoring Social Work Practicum Students.
- Demonstrate a clear understanding of the NBIS mandate.
- Adhere to the policies and procedures of NBIS.
- Attend and actively participate in all staff meetings, strategic planning meetings, training sessions, and other society activities.
- Prepare and submit all statistics, reports, time sheets and assignments in a timely fashion.
- Respond promptly and appropriately to any emergency situations that may arise.
- Adjust to changing demands quickly, reprioritize tasks as needed.
- Demonstrate effective communication skills by conveying necessary information accurately, listen effectively and ask questions when clarification is needed.
- Assist in the development of detailed lists of community resources; maintain personal reference list of relevant community resources, referrals, networks, events, etc.
- Maintain and update personal knowledge of brain injury research; gather educational materials/information packages to provide to clients.
- Assist with other functions as assigned by management.
**Required Qualifications, Skills and Attributes**
- Minimum BA in Social Work or related field and one-year experience providing front-line point of contact in social services setting.
- A deep understanding of the dynamics of gender-based violence and ability to work with female survivors of intimate partner violence and brain injury.
- Strong ability to deliver services with kindness and compassion.
- Ability to interact well with clients in crisis regardless of ethnic, religious, or socioeconomic status, sexual orientation, age, physical or mental ability.
- Ability to assess clients’ immediate needs and assist them to define and implement an action plan.
- Ability to work both independently and within a team, to take initiative, and to prioritize work.
- Ability to multitask and work with a calm and professional demeanor.
- Aware of, and sensitive to, cultural, ethnic and economic diversity; skilled at handling clients who are experiencing challenges related to mental health and/or substance misuse.
