**Key Responsibilities**:

- Case Management involving professional resources management, team management, goal setting, case budgeting and third-party funding approvals.
- Setting rehabilitation goals in consultation with clients, their families, treatment teams and other treating health professionals.
- Facilitating medical referrals and evaluations.
- Supporting communication and correspondence among all stakeholders. Key Qualifications
- Certification with the College of Nurses of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers
- Demonstrated knowledge of best rehabilitation practices and case management principles
- Clinical experience with amputations, spinal cord injury, and traumatic brain injury is considered an asset
- Superior interpersonal relations, communication, problem-solving, and time management skills
- Demonstrated ability to work collaboratively as a member of a multidisciplinary team
- Self-motivated and self-directed, with demonstrated ability to manage a busy and varied caseload
- Demonstrated knowledge and experience with the Statutory Accident Benefits Schedule (SABS)/ Motor Vehicle Accidents and the Ontario Legislation.
