**SOCIAL WORKER**

**SUMMARY**

The Smiths Falls Nurse Practitioner-Led Clinic (NPLC) provides exemplary comprehensive primary health care services to the residents of Smiths Falls and the surrounding counties of Leeds, Grenville, and Lanark.

Under indirect supervision, and working as part of an interdisciplinary team, the Social Worker provides high quality professional counseling and social navigation support to clients, consistent with the philosophy of the Smiths Falls Nurse Practitioner-Led Clinic.

**ACCOUNTABILITY**

The Social Worker reports to and is responsible to the management team. All clinical decision making outside the scope of the Social Worker must be done in consultation with the Smiths Falls NPLC medical directives. The Social Worker must comply with the standards of practice guidelines of the Ontario College of Social Workers and Social Service Workers.

**ROLES AND RESPONSIBILITIES**:

- Interview clients individually, in families, or in groups, and exercise professional judgment in assessing their situation and problems and determine the types of services required
- Provide counsel and therapy to assist clients in developing skills to deal with and resolve their social and personal problems
- Plan programs of assistance for clients including referral to agencies that provide financial assistance, legal aid, housing, medical treatment, and other services to care for the needs of the clients
- Monitor client goals and outcomes through ongoing assessment and documentation of treatment progress
- Maintain communication with clients and families to ensure follow-up on referrals
- Acts as an advocate for clients and their families in matters affecting health
- Liaises with Nurse Practitioners, and other NPLC team members related to clients’ needs as required
- Investigate cases of child abuse or neglect and take authorized protective action when necessary
- Enthusiastic, passionate, and self-driver who will participate in data management and measurement of quality improvement indicators of client outcomes
- Provides communication to hospital and community staff and other interdisciplinary team members as required ensuring smooth transition for between care sites
- Maintains clear, confidential and concise records
- Participates in continuing education for professional growth and ensures skills are kept current, and disseminates pertinent knowledge to all staff
- Teaches other health care professional students on the role of the social worker and psycho-social issues facing clients and families
- Acts as a professional and knowledgeable role model or mentor to colleagues and students
- Maintains own continuing education to enhance professional knowledge and growth
- Perform house calls and community outreach visits
- Maintains confidentiality in the work place and within the community
- Performs miscellaneous job-related duties as required

**QUALIFICATIONS**
- A bachelor’s degree in social work from an accredited university or a Social Service Worker Certification from an accredited college is required; plus 3 years of related working experience
- Must be a member in good standing with the Ontario College of Social Workers and Social Service Workers
- Previous related client care experience, specifically in a primary care setting
- Experience in solution focused brief trauma
- informed therapy, cognitive behavior therapy, dialectical behavior therapy, motivational interviewing and mindfulness
- Knowledge of the community and the resources available, especially in the areas of Crisis services and Mental Health
- Ability to operate with the highest level of confidentiality and discretion
- Strong conflict resolution skills
- Strong interpersonal skills
- Group facilitations skills
- In depth knowledge of chronic illness care, including both clinical and supportive care
- In depth knowledge of the local health program delivery and community based services
- Ability to prioritize, manage time effectively and be flexible in a very active work environment
- Demonstrated computer skills related to electronic documentation software
- Ability to work flexible hours
- Ability to travel throughout Smiths Falls and the surrounding counties of Leeds, Grenville and Lanark
- CPIC required
