**SALARY: $44.67 (MIN) TO $57.91 (MAX)

**QUALIFICATIONS **

Registered and a member in good standing with the Ontario College of Certified Social Workers
and Social Service Workers (BSW or MSW) 

Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (recent
within the last twelve months) 

Current Non-Violent Crisis Intervention (NVCI) Certificate required (training delivered by Health
Centre upon hire) 

Minimum 3 years of experience with adult and paediatric populations required 
Excellent interpersonal, interviewing and counselling skills required 
Knowledge of adult teaching and learning principles combined with recent experience in

facilitating groups required 

Knowledge / experience in education curriculum development, implementation and evaluation
required 

Excellent demonstrated ability to prepare and present educational materials designed to meet the
needs of clients, professional groups as well as community groups required 

Excellent demonstrated flexibility, organizational and leadership skills required 
Demonstrated commitment to ongoing education is required 
Self-directed professional who functions with mínimal supervision required 
Demonstrated team player required 
Critical problem solving and decision making skills required 
Excellent demonstrated ability in the areas of program development, implementation,

management and evaluation required 

Diabetes knowledge base required 
A willingness to travel; a vehicle and valid driver’s license required 
Practical knowledge of relevant legislation required 
Good knowledge of community resources required 
Critical problem solving and decision making required 
Excellent written and verbal communication skills required 

Good attendance and work record required 

Criminal Reference Check including the Vulnerable Sector Check recent within six (6) months is a

requirement
