**Team Lead Social Worker Opportunity at Hospice Niagara**

**Salary**: $54,083 - $72,110 annually
Union: SEIU Healthcare Local 1
Reports To: Manager, Community Programs

Hospice Niagara is hiring a Social Worker to be a Team Lead on our palliative community outreach team to provide counselling support for the psychological, emotional and spiritual well-being of people who have been diagnosed with a progressive life limiting illness, their partners and families from the time of diagnosis, through end of life and in bereavement.

Our Counsellors provide a wide range of interventions including individual, couple and family counselling/ support, advocacy, education, access to community supports, and spiritual companioning. In addition to the Psychosocial Bereavement Counsellor duties, the enhanced Lead role will:

- Provide clinical support to team members
- Facilitate clinical learning opportunities, including organizing case presentations, article reviews, presentation of various models of practice
- Lead community education related to death, dying, grief and loss, including development of material and co-leading sessions
- Lead and facilitating group therapy sessions, as required
- Participate in special committees, as required

This position:

- generally works Monday - Friday, 8:30am - 4:30pm
- starts at 3 weeks paid vacation
- eligible to participate in group benefit and RRSP matching plans (after waiting period)
- Master’s Degree (preferred) in Social Work and current OCSWSSW registration
- Training and experience specializing in mental health (preferred)
- 2 years of related experience in health care settings
- Expert knowledge in hospice palliative care, end-of-life care, and oncology
- Vulnerable Sector Police Check (favourable, within past 12 months)
- Fundamentals of Palliative Care (must complete within 12 months of hire)
- Bilingual in French and English (preferred)
