Healthcare Delivery Coordinator

HealthOne?is expanding and seeking a full-time?Healthcare Delivery Coordinator to join our team We are seeking a qualified professional who is impassioned by wellness, and inspired to support excellence in multidisciplinary healthcare delivery.

**Job Summary**

The Healthcare Delivery Coordinator will lead and optimize healthcare services across all HealthOne sites by establishing and maintaining high standards of care; coaching and staff recognition; oversight of the clinician journey; measuring success; crafting and delivering impactful messaging; and fostering a supportive culture for interprofessional communities and models of care. This role involves implementing a reward framework, enhancing interdisciplinary awareness and referral systems, and contributing to the development and implementation of healthcare programs and services to ensure healthcare delivery excellence at HealthOne.

**Schedule**
- Full-time (37.5 hours) Monday - Friday within the hours of 10:00am - 6:00pm
- Occasional evenings & Saturdays may be required due to the nature of the business

**Responsibilities**
- Lead, standardize and optimize the level of service excellence across healthcare departments to ensure consistent high-quality care delivery
- Act as Project Manager in the development and implementation of new Programs of Care
- Enhance interdisciplinary awareness and collaboration by establishing effective referral frameworks and communication channels
- Coach and recognize staff, oversee the clinician journey, and measure success in healthcare delivery
- Foster a supportive culture for interprofessional communities and models of care
- Implement a reward framework to incentivize excellence in healthcare delivery
- Oversee the implementation of these initiatives, tracking and measuring their impact on patient care and staff engagement
- Collaborate with various departments to ensure alignment and seamless integration of interdisciplinary efforts.
- Continuously assess and adjust strategies to optimize service excellence, reward systems, and interdisciplinary relationships to support diverse care models, operational efficiency, and improved patient outcomes

**Compensation**
- $70,000 to $90,000 per year

**Requirements**:

- Bachelor's degree in Healthcare Administration, Public Health, or a related field
- PMP designation and/or related project management experience
- Proven experience in a healthcare setting, preferably in a coordination or managerial role
- Excellent communication and interpersonal skills to interact effectively with healthcare providers, patients, and insurance companies
- Knowledge of healthcare regulations and compliance standards
- Proficiency in medical terminology and understanding of healthcare procedures.
- Solid work ethic, organizational skills and an ability to manage competing priorities effectively while remaining efficient in your delivery
- Self-starter, motivated by goals, and eager to wear multiple hats to be able to grow with us
- Openness to feedback, coaching and strategic planning to improve the practice, attain and manage KPIs, and reach business goals
- Tech savvy and proficient with MS Office (MS Word, Excel, Outlook)

**Nice-to-Haves**
- EMR experience
- Knowledge and experience with OHIP and insurance billing processes
