Community Wellbeing Facilitator

**ACCOUNTABILITY**
The Community Wellbeing Facilitator will be directly accountable to the KO Health Director
**RESPONSIBILITIES AND DUTIES**
- Travel to communities on regular basis - minimum 20 trips/year.
- Lead the implementation of Mustimuhw community electronic medical record (cEMR) system in the KO communities (to improve communication, data collection and analysis, and health programs planning and evaluation)
- Provides IT and administrative support to cEMR users in the communities
- Liaison with communities and Mustimuhw Information Solutions, and KNet (Information Technology Department) to troubleshoot challenges or barriers arising in the implementation or ongoing use of cEMR
- Provides written and verbal updates to the KO Health Director on the progress of implementation and any barriers/challenges
- Works with the Tribal Councils and communities to identify priority areas within Approaches to Community Wellbeing and focuses on those areas to develop the system.
- Supports Covid pandemic related activities - rapid testing, vaccination roll out, education, mental health support etc.
- Works with Tribal Council and communities to develop funding proposals, job descriptions, structure, work plans, and training needs at the community level.
- Develops reports and recommendations summarizing community wellbeing activities
- Provides assistance when required and any other duties assigned.
**QUALIFICATIONS**
- Minimum Grade 12
- Certificate/Diploma in a health related field
- Previous experience working in health and/or health planning
- Understanding of Health Systems (federal, provincial, and First Nations primary and public health systems);
- Computer skills
- Must be willing to travel to KO remote First Nation communities
- Knowledge of public health;
- Ability to develop and maintain respectful/ cooperative relationships with First Nation members, communities, Tribal Councils, and service providers to promote an integrated seamless delivery of service;
- Facilitation and presentation skills;
- Experience working both independently and in a team-orientated, collaborative environment is essential;
- Strong written and oral communication skills;
- Strong interpersonal skills;
- Superior time management and organizational skills;
- Ability to effectively prioritize and execute tasks in a high-pressure environment.
