Community Youth Support Worker

**Located in Picton, ON we’re looking for a Full Time Community Youth Support Worker to join our growing non-profit. If you want to develop your DS career and be actively involved with local schools and colleges, look no further than Community Living Prince Edward.**

As a Full Time Youth Support Worker, you’ll support youth and young adults who live with an intellectual disability to make successful transitions from school to post-secondary life. In this empowering position, your work will support young people to succeed in employment, volunteering, education, leisure, social, skill development as well as transitions to adult services and greater independence.

**About You**:
You are a successful collaborator who has at least two (2) years of experience working with youth who live with an intellectual disability and their families, along with two (2) years of planning and service delivery experience. In this permanent, Full Time role with Benefits, you’ll report to the Supervisor, Supports and Services. An overview of your responsibilities can be found below:
**Your Skills**:

- Advocating for others, creative problem solving & decision making, collaboration, fostering independence in others, initiative, interpersonal relations & respect and personal resilience.
- Prompt, effective and relevant service delivery to the person’s and their family’s desires and promotes respect, safety and dignity for an improved quality of life.

**Your Responsibilities**:

- Liaising and advocating on behalf of youth and their families with their school; supporting the transition from high school to community; facilitating inclusion, etc.
- Facilitating Transition Solutions Planning and enabling youth and young adults to gain lived experience in preparation for community life after school.
- Creating and implementing special interest and educational sessions curriculum.
- Facilitating Ambassador for Inclusion Training with student leaders, school personnel and community partners.
- Connecting with businesses and community partners to facilitate inclusive employment, volunteerism as well as social roles for youth and young adults inclusive of providing guidance and coaching supports.
- Coordinating with your supervisor to develop and implement employment services and training opportunities such as Students at Work and Co-op with a Purpose.
- Fostering independence and promoting the development of independent living skills and facilitating a variety of Workshop Series.
- Facilitating and actively supporting people to connect to services and community resources based on personal preference and needs.
- Contributing to building community awareness about the needs of people with disabilities and/or Community Living Prince Edward by giving presentations, attending resource fairs, forums and creating displays, etc.

**What we’re looking for**:

- Graduate of the Child and Youth Worker Program, Social Service Worker, Direct Support Worker (DSW) or other relevant Human Services degree/diploma
- An understanding of Developmental Services Ontario, ODSP, Passport Funding and other funding mechanisms
- A valid Full G driver’s license and access to a reliable vehicle
- Excellent interpersonal skills
- Ability to work both independently and as part of a team
- Current First Aid, CPR level C and CPI certificates
- Adaptability to a flexible work schedule which will include evenings and weekends in keeping with the needs of the people and families we serve
- Able to present confirmation documentation of being fully vaccinated against COVID-19 (2 doses)
