**Position Description**:
Reporting to the Director of Mental Health and Addictions, the AHT/YITW worker shall in conjunction with staff, deliver culturally safe and congruent education and care to the member First Nation communities in the Treaty #3 territory as it pertains to anti-human trafficking, and shall deliver culturally safe, trauma informed support to Aboriginal youth transitioning out of the child welfare system and into adulthood. The AHT/YITW Program will extend non-financial support to youth ages 16 to 24 (inclusive). Youth will be connected to educational, employment, housing, life skills, mental health and other support in their communities and given assistance to navigate the transition from care to adulthood. This position includes administering all aspects of the AHT/YITW program within the policy guidelines of the Kenora Chiefs Advisory and the mandate outlined by funders. The Program Coordinator will work within the MHA program to help deliver and improve the services offered to the relevant communities.

**Reports to**: Director -Mental Health and Addictions

**Duties and Responsibilities**

Ø Provide case management and work one-on-one human trafficking victims and survivors to facilitate access to resources required (i.e. counselling, housing, etc.);
Ø Provide a safe and open space for victims to discuss sexual trauma and exploitation, and practical assistance and referrals to a wide range of programs, services and counselling.

Ø Provide mentorship, advocacy and life skills development through one-on-one support;
Ø Facilitate referrals to relevant service delivery partners and to cultural healing;
Ø Assist Anti-Human Trafficking Project Coordinator with delivery of education sessions in KCA’s member communities;
Ø Develop and facilitate workshops for human trafficking victims and survivors and their families, in consultation with human trafficking survivors;
Ø Provide crisis intervention when required;
Ø Collect and maintain data required for the monitoring and reporting;
Ø Contribute to the evaluation of service with respect to its effectiveness in meeting the needs identified via community consultations;
Ø Ensure that program eligibility criteria are met

Ø Help eligible youth develop and achieve the goals they have set for themselves through the program

Ø Facilitate individual or group programming to help youth with life skills and daily living

Ø Support eligible youth leaving care to identify, access and navigate adult service systems relevant to their needs and transition to adulthood

Ø Facilitate access for youth leaving care to existing supports and resources in the community, by providing information and referrals for individual youth that address individual needs and support/reinforce strengths. Some of the resources and supports may include:

- Housing supports
- Education supports
- Employment services and training
- Life Skills training (e.g. financial management, household management)
- Health and mental health services
- Legal services

Ø Support eligible youth leaving care to develop and maintain relationships with responsible, caring adults in the community to help them as they transition to adulthood

Ø Foster communication and linkages with community agencies/organizations to improve access and mitigate barriers to services for youth leaving care

Ø Work with local CFS Agencies to develop a referral approach and to create awareness of the program among youth leaving care

Ø Participate in the reporting and sharing process, as needed.

**Qualifications**:
Ø University degree or college diploma from a recognized post-secondary institution (Psychology or Social Work, preferred);
Ø Experience coordinating and delivering programming in the area of Mental Health and Addictions, and /or Social Service fields;
Ø A combination of training and /or experience with counselling children, youth and families;
Ø A combination of training and /or experience with crisis intervention;
Ø Excellent computer skills;
Ø Knowledge of the Anishinaabe cultur
