**PURPOSE**

The Manager, Neighbourhood Primary Care and Population Health, reporting to and working closely with the Senior Director, Population Health and Community Initiatives, will oversee the on-going design, implementation, evaluation and partnership management of an innovative and multi-pronged primary care initiative with the goal of providing low-barrier and timely access to interprofessional, team-based primary care for structurally marginalized and vulnerable patients and their families, across the Region of Peel.

In partnership with local family health teams, family health organizations, CMHA, and through interprofessional populations health clinics, the Manager will oversee the staffing, coordination, development, quality improvement and sustainability of multiple primary care and allied health supported programs and teams and is responsible for the supervision, guidance, coaching, scheduling and motivation of various interprofessional primary care team members and their clinics. With a focus on high quality clinical care and harm reduction for vulnerable and structurally marginalized populations, the manager is responsible for overseeing patient safety and experience, and the successful execution of accountabilities of programs and services. The Manager ensures efficient and effective service delivery and effective approaches to population health through ongoing planning, monitoring and evaluation of its relevant programs and services. This includes program and service development, and community and partner relations and alignment across WellFort’s programs overall by working closely with the broader WellFort leadership team. This position establishes and leads inter-professional teams, cultivates strong community relationships, and integrates the work of community-based clinical care with the broader WellFort strategies and priorities.

In collaboration with the Senior Director of Population Health and Community Initiatives, the Manager will foster a culture that embraces patient-centred care, evidence-based practice, innovation, continuous quality improvement, interprofessional collaboration and care that meets the needs of equity-deserving populations. The Manager, Neighbourhood Primary Care and Population Health will be accountable for the overall performance of multiple primary health care initiatives including Central intake, the Primary Care Neighbourhood Network and Population Health Clinics and other new and evolving programs.

**RESPONSIBILITIES**
- Plan and coordinate clinical services in spaces inside and outside WellFort’s physical locations (including co-located with other primary care practices)
- Liaise with community partners and clinical leadership to ensure effective and efficient implementation of primary care models and team-based care
- Supervise teams of primary care, nurses, social workers, dieticians, physiotherapists, and other allied health professionals, including supporting professional development and performance improvement
- Ensure implementation and sustainment of policies and practices
- Maintain budget and track expenditures/transactions.
- Form action plans to ensure program quality assurance.
- Set clear goals for the teams in alignment with medical directives, organizational priorities and funder expectations and leads by setting a good example and engages the team to achieve goals and expected program delivery targets.
- Identify gaps in service and presents recommendations.
- Participate in monitoring quality indicators including client satisfaction and reporting requirements, as necessary.
- Participate in the development of department specific protocols, through PDSAs and quality improvement initiatives across the center.
- Ensure clinical practice supports to maximize all roles scopes of practice
- Develop policies and procedures to support staff in high quality care delivery and ensure appropriate resourcing within budget to meet client and staffing needs

- Work collaboratively with teams to provide education and support to community members and service providers.
