We currently have an exciting opportunity for a **Vascular Access Coordinator **to join our** Vascular Access Program. **This role is responsible for coordinating patient access to vascular and peritoneal access services in compliance with Ontario Rental Network (ORN) funding and role requirements.

**Responsibilities**

Reporting to the Director of Nephrology, the Vascular Access Coordinator will:

- Identify rationale for access type among prevalent patients and facilitate accurate reporting of data within the Online Records Retrieval System (ORRS) tracker tool
- Collaborate with patients to identify modality for body access and navigate steps
- Troubleshoot issues with body access; monitor and address with patients; coordinate actions to ease clots and maintain access method in collaboration with radiology, physician
- Report on quality improvement initiatives implemented and related progress to the Ontario Renal Network (ORN)
- Lead a team to implement and evaluate the efficacy of local quality improvement directives to decrease catheter rates and achieve appropriate access (e.g. fistulas, grafts, peritoneal access vs. central line and catheter, which have higher infection rates)
- Coordinate the delivery of quality care and education in accordance with the ORN defined clinical responsibilities (e.g. reduce risk of clotting, education for patients/families)
- Collaborate with peers and adapt proven quality improvement change concepts to drive local improvements toward stated targets

**Requirements**:

- Undergraduate degree in Nursing required
- Current certificate of registration from the College of Nurses of Ontario.
- Current certification with the CVAA (The Canadian Vascular Access Association).
- Membership with Nurses Association of Ontario (RNAO) preferred
- Nephrology Nursing - RN certificate required
- Minimum 5 years experience in Nephrology (home dialysis) required
- Knowledge and applied experience in Home hemodialysis and peritoneal dialysis; vascular access sites and organ transplants
- Strong interpersonal skills, both oral and written
- Ability to empathize and build rapport
- Computer proficiency including Microsoft Office Suite
- Excellent attendance and discipline free record required.
