**Job Description**:
We are seeking a dedicated and compassionate Social Service Worker/Admissions Coordinator to join our team. In this role, you will be responsible for facilitating admissions, assessments, and providing social work support for our residents. This position requires strong interpersonal skills, empathy, and a commitment to improving the lives of those in our care.

**Responsibilities**:

- Conduct comprehensive assessments of residents to determine their social and emotional needs.
- Collaborate with healthcare professionals to develop individualized care plans for residents.
- Assist with the admission process, ensuring all required paperwork and documentation are complete.
- Provide emotional support and counseling to residents and their families.
- Advocate for residents' rights and ensure they receive appropriate care and services.
- Coordinate and participate in planning and referrals to appropriate community resources.
- Maintain accurate records and documentation of all interactions and assessments.
- Stay updated on social service work best practices, regulations, and trends.

Qualifications:

- College diploma in Social Service Work is required.
- Proven experience in social service work and admissions coordination within a healthcare setting is a plus.
- Excellent communication and interpersonal skills.
- Strong organizational skills and attention to detail.
- Empathy, patience, and a commitment to helping others.
- Knowledge of relevant laws and regulations in healthcare and social service work.

**Salary**: From $1.00 per hour
