**Qualifications**
- Current registration with British Columbia College of Social Workers (BCCSW)
- Minium of 3 years post Bachelor of Social Work experience working with mental health challenges in community settings.
- Broad knowledge of legislation and government policies and procedures that relate to senior residents.
- Demonstrates ability to establish rapport with residents and families in order to explore issues that are beyond the initial presented concerns.
- Ability to communicate effectively both verbally and writing with residents, families, team members, and other community personnel.
- Ability to adjust to changes, unexpected events, and effectively deal with difficult situations.
- Proven ability to provide staff education and mentorship.
- Strong knowledge of community resources and referral systems.
- Demonstrates excellent communication, conflict resolution, and leadership skills.
- Physical ability to perform the duties of the position.
**Duties**
- Provides direct social work services and counseling to resident and families relative to social, environmental and emotional problems precipitated by illness, death, family related issues, financial, disability and hospitalization.
- Develops and prepares written social assessments on all new residents to describe the social and emotional factors pertinent to the residents’ situation and evaluate the need for services.
- Acts as an advocate for the family and the facility to prevent and reduce conflict.
- Provides consultation to staff, community agencies, and administration related to social work services.
- In consultation with the Director of Care, develops, implements, and monitors social work service programs, policies, procedures and quality improvement and risk management activities.
- Maintains a current knowledge of, and complies with Seton Villa’s visions, values, policies and procedures.
- Attends the inter-disciplinary meeting and provides an update on intervention with residents and families.
- Maintains clinical records.
- Records information for Quality Indicators monthly.
- Builds and maintains positive, collaborative relationships with residents, families, and staff.
- Communicates risk management issues to Director of Care and participates in controlling and reducing risks.
