Social Worker, In-Patient Psychiatry Fraser Health Authority Salary CAD $42.27 - $52.81 / hour Job Summary Join our team and empower lives through social work You will provide clinical social work counseling, conduct assessments, support families, coordinate care plans, and collaborate with an interdisciplinary team in an inpatient psychiatric setting. Responsibilities Provide comprehensive therapeutic counseling services to patients and families, coordinating with treatment teams and community supports. Conduct comprehensive social work and psychosocial assessments for patients with mental illness and substance misuse. Educate, consult, and support patients, families, and healthcare professionals on high‑risk factors, recovery, community practice, and relevant areas (child protection, chemical dependency, etc.). Maintain accurate patient records, charting, progress notes, and reports in accordance with policies. Plan and facilitate discharge planning, coordinating care during hospital stay and transitions to community services. Arrange and participate in interdisciplinary team meetings, community service referrals, and liaison with mental health agencies. Develop, coordinate, and deliver in‑services, presentations, psycho‑educational and support groups. Initiate, develop and carry out research activities and program planning relevant to patient and community needs. Maintain current knowledge in social work practice, reviewing literature and participating in professional development. Perform other related duties as assigned. Qualifications Master's degree in Social Work from an approved school of Social Work. At least two years of recent, related clinical experience in a mental health treatment setting. Current full registration with the BC College of Social Workers. Skills and Abilities Comprehensive knowledge of emotional issues related to mental illness and addictions, and patient/family cultural dynamics. Advanced counseling skills, including crisis counseling, brief therapeutic interventions, and group work. Ability to organize and prioritize workload in a complex environment. Proficiency in education and consultation to patients, families and healthcare professionals. Leadership in interdisciplinary care planning and effective utilization of patient resources. Effective referral skills, knowledge of community resources, and collaborative skills with community health and social agencies. Effective communication skills, both verbal and written, with patients, families, staff and community agency personnel. Ability to develop, coordinate and deliver in‑services, presentations, psycho‑educational and support groups. Ability to initiate, develop and carry out research activities.
