**Overview**
**Duties**
- Conduct assessments to determine the needs of clients and develop appropriate care plans.
- Provide case management services, ensuring clients receive the necessary support and resources.
- Collaborate with other professionals to coordinate care and address any social issues.
- Maintain accurate records of client interactions, progress, and outcomes for data collection and research purposes.
- Advocate for clients' needs within multiple systems and connect them with community resources.

**Requirements**:
- Bachelor’s degree in Social Work or related field; OR diploma in a related field: MUST be eligible for registration with an appropriate counselling body
- Strong understanding of case management principles and practices.
- Excellent communication skills, both verbal and written, with the ability to engage effectively with clients, families, and professionals.
- Proficiency in data collection methods and research practices related to social work outcomes.
- Ability to work collaboratively within a multidisciplinary team while maintaining a client-centered approach.
- Compassionate demeanor with a commitment to advocating for the well-being of clients and their families.

Pay: $30.00-$35.00 per hour
