Our Community Support Services (CSS) team provides essential services that enable our clients to thrive. Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.
**The opportunity**
We are currently looking to fill a **temporary Part-Time Community Services Resource Worker (CSRW) role in Hamilton and Burlington**.
**Hours**: 21 hours per week
**Responsibilities**
- Complete interRAI CHA Assessment for new and existing clients.
- Intake and referral process for new clients.
- Liaise with Home and Community Care Support Services and other community based agencies
- Wait list management and maintenance of files to ISO standards.
- Support wait list applicants to access community resources.
- Act as a community and intra-agency liaison, promoting a positive image of March of Dimes Canada and our services in the community.
**Qualifications and experience**
- Post secondary education in a related field (health or human services, PSW, SSW, DSW, or related) consider an asset.
- Experience and/or knowledge of interRAI CHA Assessment tool considered an asset
- Experience with completing intake assessments considered an asset
- Current Standard CPR/First Aid Certification or ability to renew
- Proficient in computer knowledge.
- Demonstrated ability to maintain confidentiality.
- Ability to communicate using a variety of methods, including sensitive conversations and conflict resolution with consumers
- Demonstrated reliability, maturity, ability to work independently, and strong organizational skills.
- Knowledge of assistive devices, funding sources, community services and resources, and assessment tools.
- Valid Driver’s License and insurance, and their own reliable transportation is required. Applicant must be willing to travel throughout the Region.
