**Team Lead - Hospital to Home**
**(**_Full-time position, 35 hours per week, Monday to Friday 8:30 A.M. - 4:30 P.M. with occasional work outside of business hours.) _**
**The Role**
Reporting to the Integrated Care Manager, Hospital Programs, you will support CHATS Hospital to Home Program with the goal of optimizing team and individual functions including but not limited to on-boarding of new team members, supporting access and orientation to key IT platforms as well as providing leadership on documentation, communication and collaboration amongst partner agencies and within CHATS. You will complete client care needs assessment (independently or with a partner agency) and provide caregiver support and community navigation.
**Key responsibilities of the position**:
- Provide leadership in the areas of documentation and communication.
- Support the onboarding of new team members
- Provision of timely and comprehensive client needs assessments with a focus on safety and risk and ongoing case management and care planning.
- Provide direct service to caregivers including assessment, wellness education, training or information services, and maintain integrity of client records.
- Foster positive relationships with Ontario Health Team (OHT) partners; Ontario Health at Home and other referral sources to promote service awareness, increase referrals to CHATS services, identify and build service opportunities.
**What you bring to the role**:
- Social Service Worker-Gerontology or Degree in Gerontology or related education, combined with three years’ experience with seniors and/or provision of client and/or caregiver services in the community, including program development.
- Excellent assessment, interpersonal communication and problem-solving skills.
- Excellent verbal and written communication/documentation skills.
- Excellent skills in risk, crisis and conflict management.
- Experience working in multi-partner initiatives is preferred.
- A valid Ontario drivers’ license and a vehicle are required for this position.
- Ability to accommodate a flexible work schedule which may include occasional evenings and weekends.
- Current certification in First Aid and CPR.
- Fluency in both official languages (English & French) and/or other language(s), an asset.
