**KEY DUTIES AND RESPONSIBILITIES INCLUDE**:
- Performs all functions in accordance with the Mission, Vision and Values and Administrative policies and procedures of St. Joseph’s Villa care programs
- Liaison between admissions and finance to help coordinate and collect necessary information and signatures / consents, including but not limited to: Admission notice must show actual time finance team is to meet with family/resident and if any delays are expected, confirm resident/family is bringing required documentation (E.g.: Notice of Assessment, Service Canada letters, POA for property, VOID cheque etc.), contact finance prior if there is a problem, communicate to upcoming admissions what can be done prior to admit day, such as involuntary separation, knowledge of the incoming resident’s age, marital status, possible dependents, provides finance the information required to compile the proper admission package, and explain rate reduction program, eligibility and required paperwork
- Responsible for referring residents to BSO (Behavioural Supports Ontario) at the point of admission based on risk screening
- Participates on committees, working groups and teams as appropriate, approved or directed
- Maintain community and mental health resources and connections for residents, networking with community social workers and programs to develop partnerships that support residents.
- Connect family members with community resources to support their personal needs and required supports.
- Provides goal-based counselling to residents who are cognitively capable of participating in the process.
- Provides clinical services to residents and their families/significant others as required or requested. This includes but is not limited to biopsychosocial assessments, counselling, problem solving and making connections for future services within community and development of resident and staff safety plans.
**FORMAL EDUCATION**:
- M.S.W. or B.S.W degree from an accredited School of Social Work and current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker in good standing
- Recent (3-5 years) experience providing individual, client and family support services
- Demonstrated knowledge of behavioral and suicidal risk assessment, crisis management, and verbal diffusion techniques
- Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act, and community mental health resources
- Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case management and advocacy skills
**OTHER KNOWLEDGE, SKILLS, AND EXPERIENCE**:
- Experience in a health care setting and social service work with care of seniors and clients
- Knowledge in Long Term Care Homes Act, Rules and Regulations
- Knowledge of Specialized programming for care and therapy in community
- Knowledge of Community Services and referrals processes
- Knowledge of current development and research in Gerontology
