**Job Title**: Grief & Bereavement Practitioner

**PURPOSE OF THE ROLE**:
Reporting to the Director of Stedman Community Hospice, and guided by SJLCB’s Mission, Vision and Values, the GBP operates as a member of the multidisciplinary team championing patient centered care in-house and with the Outreach programs. In collaboration with department colleagues, ensures prompt triage of referrals, completes assessments and makes appropriate referral to hospice programs and services. Works collaboratively with community partners to ensure patient/client needs are adequately met based on individual needs assessments. Will act as a champion of grief & bereavement both internally and externally, fostering ongoing and new partnerships with community stakeholders i.e. local funeral homes, mental health resources, community health centers and colleagues in the hospice palliative care field.

**KEY ACCOUNTABILITIES & SUCCESS** CRITERIA**:
**Psycho-Social and Emotional Support**
- Conducts comprehensive psycho-social assessments to provide holistic, person-centred care.
- Offers emotional and practical support to individuals, families, and caregivers, including children and youth, facing life-limiting illness throughout the entire grief process.
- Supports staff through debriefings and shared reflections to foster team resilience.
- Responds effectively to crises, using sound judgment under stress.

**Grief and Bereavement Care**
- Delivers bereavement support through individual counseling, group facilitation, and community referrals.
- Co-facilitates bereavement groups and organizes grief-related community events.
- Applies evidence-based grief models and tools in assessments and interventions.

**Education and Professional Development**
- Develops and delivers educational programs for staff, volunteers, students, and the community.
- Engages in continuous professional learning and contributes to policy and practice improvements aligned with national standards.

**Interdisciplinary Collaboration**
- Participates in care planning, team rounds, and care conferences to address patients' and families' supportive needs.
- Coordinates services with internal teams and external providers to ensure integrated care.
- Supports and oversees volunteers involved in psycho-social and bereavement services.
- Contributes to quality improvement initiatives and committee work.

**Documentation and Reporting**
- Maintains accurate and timely clinical documentation in compliance with professional standards and organizational policies.
- Prepares administrative reports and participates in program evaluation and outcome measurement.

**Community Engagement**
- Builds and maintains partnerships with community organizations and service providers to enhance hospice care delivery.
- Acts as a liaison with external stakeholders such as funeral homes and mental health resources.

**QUALIFICATIONS & SKILLS**:

- Bachelor’s degree in Social Work, Psychology, or a related field
- Master’s degree an asset
- Registration in good standing with the appropriate regulatory college.
- Specialized training in Thanatology, Palliative Care, or related fields is an asset.
- Demonstrated commitment to Equity, Diversity, Inclusion, and Indigenous Reconciliation.
- Strong communication, active listening, and critical thinking skills; effective in both individual and group settings.
- Maintains high standards of confidentiality and professional boundaries while remaining compassionate and empathetic.
- Proven ability to work independently and collaboratively within interdisciplinary teams.
- Experience supporting children, youth, and families through grief and loss.
- Skilled in planning, prioritization, and self-care, with healthy coping strategies for managing loss.
- Knowledgeable in palliative and end-of-life care principles and best practices.
- Experience in volunteer coordination and delivering education or training to diverse audiences.
- Valid driver’s license, a reliable vehicle for work-related purposes
