**Who We Are?**

**MEDCENTRE HEALTH** is located in the Eastern GTA, providing Patient Care / Acute Medical Care to the community, and treated more than 100,000 patients since the inception in 2012.

**What is the Project?**

Now we are involved in our biggest Project since the inception of **MEDCENTRE HEALTH**, to develop a modern (With latest digital components) Home health Care company in the GTA.

Our Home Health Care company is planning to do home health care services, home nursing services, home physiotherapy, occupational therapy services, personal care services, homemaker, and companion services at our client’s home.

Our services will benefit **anyone **who needs medical and non-medical care immediately after a hospital discharge or during an acute disability and **Seniors** who needs medical and non-medical care to live in their own home.

Are you a career oriented dynamic leader who can spend at least 3 years in Home Health Care industry by stepping into current project of creating the Home Health Care & be part of upcoming projects and coordinate the Home Health Care Program in the future?

Our central office is located in Scarborough from which operational tasks of Home Health Care (scheduling of care, ensuring records of care are managed, and service plans are administrated) are executed (from the same building our Urgent Care Centre operates).

**Responsibilities of the role**:
**(1)** **Coordinating our vital project of developing a Home Health Care organization**:
(a) Play a vital role in the Initiation Stage - Do an extensive information search about the home health care industry in Ontario.

(b) Research & study the current situation about Home Health Care in Ontario & Study the latest US models and help the “CEO/Project Lead/Product Owner” in setting the Goals.

(c) Assist accurate and timely completion of assigned tasks during the project.

(d) Track and report to the “CEO/Project Lead/Product Owner” about the project milestone status, expected completion date etc.

(e) Schedule and attend Project team meetings, Capture meeting minutes, discussion with the “CEO/Project Lead/Product Owner” and document action plan & next steps.

(f) Assist the Project Lead/ Product owner with the creation of project roadmaps, track issues and mitigation plans.

(g) Assist with documentation and creation of standard processes as needed.

(i) Event management.

**(2)** **Continue in the company as Program Coordinator**:
(a) Schedule agendas and staff shifts in accordance with the Program’s needs.

(b) Monitor the Program’s budget and maximise cost-effectiveness.

(c) Approve the purchase of necessary supplies and equipment.

(d) Manage the Program’s staff, keeping them motivated and solving any problems that may arise

(f) Strategize and implement policies and practices.

(g) Manage communication between program staff, the CEO, and relevant external stakeholders.

**Qualifications and Experience Required**:

- Degree/Diploma in Medical, Nursing, Health Care Administration or Business Administration. Internationally trained graduates also considered.
- Certification in Project Management (Minimum CAPM/DASM. PMP/PMI-ACP preferred.) or Ontario Diploma/Degree in Project Management.
- Minimum of 2 years of experience managing and leading people, preferably Project management.
- Experience using Microsoft Office Suite (including Word, Excel, PowerPoint, SharePoint, Teams).

**Ability to commute**:

- Ability to easily commute or willing to relocate near our Scarborough Central Office.

**Salary & Benefits**:

- **$50,300 - $65,800 per annum**, depending on the Health Care Project experience & Certification
- **In person work only on weekdays**. Maximum work 40-44 hours a week.
