To bring this mission to life, Supportive Homes provides both group and independent living homes along with levels of support that match the needs of those in our care. Those in our care include individuals diagnosed with developmental disabilities and those with dual diagnoses.

As for our history, our founding team combines over 20 years of experience in providing housing and care for those living with mental health conditions, 18 years of experience in supporting those living with disabilities, and 15 years of experience managing highly diverse, high-performing teams.

**Direct Support Mentor**

Your role as a Direct Support Mentor is to work independently to monitor and provide counselling to the people we support, assisting them in living life as directed by their needs and desires. As a valued team member, you'll do everything in your power to treat the people you support with dignity and the utmost respect to help make their dreams and aspirations a reality.

**Key Priorities**:

- Ability to work **independently** while providing ongoing **monitoring and counselling** to clients
- Strong **communication skills** (written and verbal) to ensure clear and effective interactions with clients and team members
- Facilitate **life skills development** in areas such as hygiene, budgeting, cooking, nutrition, and social skills
- Support individuals in making informed decisions and achieving personal goals
- Provide **community engagement opportunities**, including outings and social integration
- Assist in **conflict resolution** and manage challenging behaviours in residential and community settings

**Experience and/or Qualifications**:

- Experience supporting adults with developmental disabilities and mental health issues
- Experience with and a passion for **person-centred support** and its philosophies
- Pursuing and/or obtained a diploma or degree in a relevant discipline (DSW, PSW, CYW, BA, BST, Social Services, etc.)
- Strong communication skills, both written and verbal
- A vehicle and/or a valid G license

**Certifications & Compliance (**Mandatory to Start Work**)**
- **Valid First Aid & CPR Emergency Level C certification**:

- **Valid Crisis Prevention and Intervention (CPI) certification**:

- _UMAB (Understanding and Managing Aggressive Behaviour) may be accepted as an equivalent_
- **Current Vulnerable Sector Check (VSC) and Broad Record Check (BRC) required.**Ideally within the last 6 months.**Applicants with existing checks are strongly preferred due to long processing times.

**Responsibilities**:

- Provide **mentoring and counselling** to individuals to support their personal growth, goal achievement, and overall well-being
- Treat all people supported and colleagues with dignity and respect
- Foster independence by teaching and assisting with practical daily living skills such as cooking, cleaning, laundry, promoting self-care, and pursuing personal goals
- Deliver care according to each person’s individual support plan and contribute to the continued development of each person’s plan through documentation and collaborative communication
- Participate in and create fun, educational, and engaging activities
- Support the development and maintenance of meaningful relationships
- Escort and support individuals during community outings
- Provide administrative support as needed to maintain accurate records and documentation
