**Job Description**:
In accordance with the Vision, Purpose, and Values, and strategic direction of Island Health (Vancouver Island Health Authority), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Director, Clinical Services, the Community Response Coordinator serves as part of the senior nursing team. The Community Response Coordinator provides clinical support to the community palliative care services of the Victoria Hospice Society (VHS), including consultation with community, acute and long-term care partners, patients and families, and outpatient and in-home support for the Palliative Response Team. The Community Response Coordinator participates in the development, delivery and evaluation of hospice palliative care community services, professional and public education, and research and quality improvement initiatives. The Community Response Coordinator provides direct nursing care as required.

Travels to other sites throughout Island Health and other health care provider sites as required. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle.

**QUALIFICATIONS**:
**Education, Training And Experience**:
Graduation from a recognized nursing program. Current registration with the BC College of Nurses and Midwives as a practicing RN registrant. Certificate in Palliative Care Nursing (CNA) preferred. Valid BC driver's license.

Minimum of three (3) years recent, related experience in a specialized palliative care setting with community nursing experience, including one (1) year recent, related teaching experience or an equivalent combination of education, training, and experience. Leadership/supervisory experience an asset.

- Ability to organize, coordinate and prioritize workload effectively.
- Ability to establish and maintain effective working relationships with interdisciplinary team members, partners and external agencies.
- Demonstrated program development and evaluation skills.
- Physical ability to carry out the duties of the position.
