Location - Niagara Long Term Care Residence

**PROGRAM THERAPIST JOB POSTING**

**Essential Duties are the technical skills and knowledge that an employee requires in order to perform their role. Below is detailed information on required Essential Duties for your position.**
- **Complete initial and ongoing programs assessments as necessary to determine residents’ recreation and rehabilitation needs.**:

- **Complete documentation and accurate records as required including: Program guides, participation tracking, monthly statistics, Intake documentation, Quarterly documentation, MDS/RAPS, Care plan updates**:

- **Plan and implement programs based on resident assessments completed**:

- **Facilitate one-on-one and small group programs on unit**:

- **Facilitate or Co-facilitate large group weekly programs including religious services and entertainment performances**:

- **Plan and implement special events, community based programs and outings**:

- **Evaluate and make adaptations to programs as appropriate**:

- **Complete equipment assessments and ensure that resident has access to appropriate assistive devices as required (i.e. wheelchair, walker)**:

- **Provide most appropriate available wheelchair/walker for new admissions and current residents in response to a change in status**:

- **Track and label Rehabilitation/Recreational equipment on unit**:

- **Facilitate repair of mobility aids and seat belt installation**:

- **Provide residents with a calendar of programs offered**:

- **Ensures that posters, notices, and calendars are up to date, attractively displayed and easy for residents to read.**:

- **Required to understand the nature and meaning of quality indicators**:

- **Other duties as assigned and or indicated in Job Task Inventory
