Coordinate care between healthcare providers and ensure services are not duplicated. Monitor patient progress and make sure care meets clinical and insurance standards. Work closely with clients, providers, and internal teams to ensure smooth communication and documentation. Assist with repatriation or evacuation planning when needed. Document everything in the company’s case management system in real time. Help train non-medical case managers when required. Key Responsibilities Evaluate treatment plans, monitor hospital stays, and assess service requests. Refer complex cases to a supervisor or medical director. Support discharge planning, ensuring care is appropriate and cost-effective. Stay up-to-date on best practices and regulations like PIPEDA, GDPR, and HIPAA. Track case progress and update internal systems regularly. Attend team meetings and support quality improvement efforts.
