The Hudson Regional Cancer Program (HRCP) is one of fourteen in the province of Ontario. The Oncology Dietitian works as a key member of the HRCP interdisciplinary health care team to assess the nutritional status and needs of medically complex clients throughout the lifecycle, in the outpatient oncology setting, in order to ensure exceptional client care. The Oncology Dietitian will implement nutritional care plans, ensure the effectiveness of the plan and provide education to the patient. Education: A Registered Dietitian with a Bachelors degree and a Dietitians of Canada (DC) approved internship. Experience: Several years of clinical experience required. Oncology experience is preferred and will be considered an asset. Experience monitoring and evaluating pancreatic enzyme replacement therapy will be considered an asset. Professional Affiliation: Registered with the College of Dietitians of Ontario. Eligible for membership in the Dietitians of Canada and appropriate DC networks. Specialized areas may have further requirements, i.e. Certified Diabetes Education Certification is required for RD’s working at DMC. Responsibilities: Complete Nutritional Assessment by: Collecting information regarding diagnosis, medical history, social and cultural history, medications, weight history and anthropometrics, client expectations/goals, relevant allied health consultations, physical observations. Integrating and interpreting collected information to determine nutritional status and nutritional requirements (including macro and micronutrient requirements). Developing appropriate and individualized nutrition care plan (together with the client establishing realistic goals) and determining outcome measures for monitoring its effectiveness. Documenting nutritional assessment. Reviewing malnutrition screening tool responses. Consulting with Day 1 chemotherapy patients. Implement Nutritional Intervention by: Providing individualized nutrition education according to the established nutrition care plan and the client’s learning needs (considering level of education, client’s readiness to change, disabilities), modifying educational tools/resources accordingly. Implementing the appropriate nutrition care plan (includes communication with client/family/caregiver, diet technician and with other health care professionals within the hospital and the client’s circle of care). Monitoring and adjusting the nutritional care plan as indicated. Documenting nutritional intervention. Experience monitoring and evaluating pancreatic enzyme replacement therapy will be considered an asset. Carryout Consultation/Collaboration, as required by: Communicating professional consultation regarding individual client care with physicians, nursing and other allied health within the hospital and the client’s circle of care. Participating in team rounds, case conferences and other team meetings. Documenting consultations. Participate in multidisciplinary grand rounds to identify patients who would benefit from early intervention. Will be a Professional Resource and provide Mentoring by: Mentoring of dietetic interns. Being a nutrition resource for health care professionals and food service staff. Participating in the development of policies and procedures and clinical pathways. Providing advice regarding therapeutic menu development and foodservice procedures related to client care. Planning and executing nutrition-related events (includes nutrition month activities, health fair and other RVH events or activities). Representing RVH at OH-CCO communities of practice, advisory panels and committees, as needed. Assisting to develop and facilitate nutritional therapy group programming. Assist in writing briefing notes and business plans to support the growth of the program and novel approaches to care. Developing patient facing education. Maintain Professional Development by: Reviewing current literature to ensure nutrition practices evidence-based and reflects best practices. Attending relevant in-services, conferences and teleconferences. Participating in College of Dietitians of Ontario’s quality assurance programs including completion of the annual self-directed learning package to identify learning needs and establish professional goals. Taking courses to meet identified learning needs. Fulfill Administrative Responsibilities by: Participating in departmental and organizational meetings. Driving dietitian practice by assisting in the development of medical directives, like PERT administration. Completing workload measurement. Maintaining supplies and resources, with consideration of the approved budget. Reviewing and responding as needed to organization-specified emails. Abiding by the Code of Ethics, and Scope of Practice, College of Dietitians and the Code of Conduct and the dress code, RVH as well as act such as Regulated Health Professions Act.
