Your Opportunity:As a Social Worker-Discharge Coordinator, out of the Northern Lights Regional Health Centre, you will have the opportunity to work within a unique diverse environment and enjoy the benefits of a larger team while collaborating with other professionals. In addition to a competitive rate of pay, AHS currently has a Fort McMurray Northern Living Allowance in place to an annual maximum of $12,480. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.Description: As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities. Required Qualifications: Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).Additional Required Qualifications: Past experience with working with various agencies in the Fort McMurray area. Experience with case management and coordination.Preferred Qualifications: As required.
