Job Description The Saskatchewan Health Authority (SHA), Regina Area, invites applications for a physician with specialized training or demonstrated competence in chronic disease management. This position will provide community-based services in collaboration with Primary Health Care teams to support clients living with complex chronic conditions. The clinic will be located in Regina and will provide support across all Primary Health Care networks in the Regina area, abiding with the ministry's Patient Medical Home initiative. This role focuses on integrated, interdisciplinary care to enhance access, improve outcomes, and reduce gaps in service for individuals managing chronic diseases. Key Responsibility Collaborate with Cardiac, Respiratory, and Diabetes programs. Serve as a liaison between Primary Health Care teams, community physicians, and specialists (e.g., respirology, cardiology, endocrinology). Provide short-term support for unattached clients while long-term physician connections are established. Address service gaps caused by limited access to specialists and family physicians in the area, contributing to improved health equity. Help connect Chronic Disease Prevention and Management (CDPM) programs with local physician practices. Act as a clinical resource on chronic disease supports and services available through Primary Health Care. Lead a family physician-led model of care for managing fewer complex patients who do not require specialist care but need more support than typically provided in general practice. Approximately 80% of clients are appropriate for management in the primary care setting. Provide support with insulin adjustments and transitional care for youth moving from pediatric to adult endocrinology services. Liaise with the Home Ventilation and Respiratory Outreach programs. Strengthen interdisciplinary collaboration to enhance patient experience, reduce ER/hospital use, improve chronic disease outcomes, and support staff development and satisfaction. Team Collaboration: You will work alongside an established interdisciplinary team that includes family physicians, nurse practitioners, registered and licensed practical nurses, respiratory therapists, a pharmacist, dietitians, and administrative support. Strong partnerships across programs ensure seamless, patient-centered care. The SHA is dedicated to learning and is a partner in the growing educational needs of medical students and residents from the College of Medicine, University of Saskatchewan. The University of Saskatchewan is a research-intensive institution with 26,000 students and 6,500 faculty and staff, and a strong reputation for innovation and excellence. The College of Medicine has an intake of 100 students per year with a commensurate number of positions for residency training and a number of distributed training sites in Regina and Saskatoon. Job Qualifications The candidate will hold certification from the or from the Royal College of Physicians and Surgeons of Canada. Individuals with fellowship training in pain medicine will deserve preference. Successful applicants will be eligible for a regular license to practice in Saskatchewan from the College of Physicians and Surgeons of Saskatchewan (CPSS). The successful candidate must be licensed by CPSS to practice and prescribe initiation and maintenance opioid agonist therapy for persons with opioid dependence or be eligible and willing to obtain such licensure. This individual should be a team player, be able to interact effectively with patients, colleagues, staff and management, and function well in a high-volume environment with a commitment to provision of high quality patient and family centered care. The SHA requires a criminal record check (CRC) from every potential SHA team member. The CRC must be dated within the past six (6) months and be satisfactory to the SHA.
