**Job Overview**

**Duties**
- Conduct thorough patient assessments and evaluations to determine health status and medical needs.
- Develop and implement individualized treatment plans based on patient diagnoses and preferences.
- Provide ongoing patient care, including medication administration, pain management, and acute care interventions.
- Perform routine check-ups, screenings, and preventive health measures for patients across all age groups.
- Manage chronic illnesses and coordinate home care services as needed.
- Utilize electronic health record systems such as Epic and Cerner for accurate documentation and data collection.
- Engage in research activities to stay current with medical advancements and improve patient outcomes.
- Collaborate with specialists and other healthcare providers to ensure comprehensive patient care.

**Skills**
- Strong background in patient care with a focus on family medicine principles.
- Proficient in pain management techniques and acute care practices.
- Experience with pediatric care is preferred but not mandatory.
- Excellent data collection skills for effective diagnosis and treatment planning.
- Familiarity with electronic health record systems, particularly Epic and Cerner.
- Ability to conduct research that contributes to improved clinical practices.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Commitment to providing high-quality home care services when necessary.
