Community Support WorkerEmployment Type: Full-Time Reports to: Program ManagerOrganization: Revolve Home HealthRole OverviewThe Community Support Worker provides direct, in-person, non-clinical support to adults with disabilities living in continuing care facilities. The role focuses on helping individuals participate in meaningful community activities, reduce social isolation, and achieve personal goals identified through Individualized Support Plans (ISPs).Key ResponsibilitiesIntake & AssessmentParticipate in intake and eligibility confirmationConduct needs-based assessments under program guidelinesCollaborate with clients to identify interests, goals, and barriersIndividualized Support Planning (ISP)Develop Individualized Support Plans in collaboration with clientsReview and update ISPs regularly based on progress and changing needsEnsure client goals are respected and prioritizedDirect Client SupportProvide in-person accompaniment to community activitiesAssist clients with: TransportationSocial and recreational outingsAppointments and community engagementFacilitate participation in activities aligned with ISP goalsProvide in-facility programming when community outings are not feasibleDocumentation & ReportingMaintain accurate records of services deliveredTrack client participation and outcomesSubmit documentation to Program Manager in a timely mannerCollaboration & SafetyCommunicate effectively with care facility staffFollow safety, confidentiality, and professional conduct standardsIdentify and report risks or concerns promptly 🎓 Required QualificationsDiploma in Human Services, Social Work, Community Support, Disability Studies, or related fieldMinimum 2 years’ experience supporting adults with disabilitiesExperience with needs-based assessments and person-centred planningStrong interpersonal and communication skillsValid driver’s license and access to reliable transportationStart Date: End of February 2026If you are interested, have the qualifications and are located in Lethbridge.
